This is a clinical services position at the Florida Department of Health in Walton County providing practical nursing support and assistance to providers primarily in the primary care program but may assist in other clinical programs as needed in accordance with agency policies, procedures, protocols, and guidelines. This position is a key custodian for the medication room, laboratory and medical records room. Adheres to Information security policies and procedures. This position is based at the primary site located in DeFuniak Springs, FL but may be assigned to other clinic locations as needed for staff coverage.

 

Your Specific Responsibilities:

 

Practical Nursing Tasks

 

Direct Client Care (infants, school aged children, adults)

    • Performs Hearing and Vision Screens
    • Performs EKGs and other in house diagnostics
    • Provides phlebotomy and specimen collection services for Infants, Children and Adults
    • Assists Provider with bedside procedures
    • Administers oral, intramuscular, topical, subcutaneous, and inhalation medications.
    • Provides educational counseling and educational brochures to clients as indicated by health care needs

 

Triage and Work Up (infants, school aged children, adults)

    • Obtains and documents Vital Signs, Height, Weight, Body Mass Index (BMI)
    • Obtains Chief Complaint/History of Present Illness, Medical History, Allergy History, Medication History and reconciliation of Current Medication List
    • Prepares client / exam room for provider visit
    • Reports abnormal findings to provider
    • Responsible for efficient clinic flow by working at a speed that maximizes provider productivity and minimizes client wait times.

Documentation

    • Record Objective and Subjective Data in Electronic Health Record as required for each designated program
    • Record Objective and Subjective Data before the end of the clinic visit
    • Perform Chart Review as directed for QA/QI
    • Documents lab work in appropriate lab binders and in HMS
    • Captures client services on superbill and/or HMS at each visit
    • Documents DAP medication pick up in HMS
    • Completes HEDIS/UDS measure tool for clients prior to each visit
    • Addresses and/or completes provider driven follow up list daily

Facility Maintenance

    • Assures necessary supplies are readily available at all times in assigned clinic exam rooms and in the clinic procedure room.
    • Cleans exam rooms and direct patient care equipment after each visit and ensures clinic exam rooms are client ready before each visit
    • Empties trash and disposes of supply boxes and or containers
    • Secures full sharps containers, stores full sharp containers in designated area, and replaces containers as needed in clinic areas
    • Cleans spills immediately when observed or discovered
    • Immediately reports non-functioning or broken equipment to direct supervisor

Laboratory Duties (may function as a back-up lab nurse)

    • Collects laboratory specimens per provider’s orders (blood, urine, sputum, stool, culture swabs)
    • Labels specimens
    • Enters client data for lab order into HMS for processing
    • Stores specimens according to laboratory requirements prior to shipping
    • Use of lab equipment including centrifuge, autoclave, glucose machine, hemoglobin machine
    • Performs in house labs as ordered and documents in HMS and in services
    • Performs QA/QI lab controls as assigned

Other:

  • May perform other duties within the practical nursing scope of practice as directed.
  • Attends and participates in monthly general staff meetings and departmental meetings
  • Completes all job related assigned trainings
  • Completes Employee Activity Reports and submits timesheets according to agency standards. Performs other duties as assigned.

  • Working knowledge of current ACIP immunization schedules, use of VIS statements, FL Shots system.
  • Ability to use and maintain clinic equipment such as the centrifuge, autoclave, EKG machine, nebulizer, pulse oximeter,  
  • manual and digital scales, stadiometer, audiometer, vision screening machine, glucose machine, eye wash station,
  • electronic blood pressure machine, manual blood pressure cuff, and HgbA1C and hemoglobin machines.

 

Qualifications:

Minimum – Active/Clear Florida LPN license. BLS Certification, current state issued driver's license, and HIV 501 certification

Preferred – 1-2 years experience as a LPN in Primary Care or in a Public Health Role, Active/Clear Florida LPN license. BLS Certification, current state issued driver's license, and HIV 501 certification
