This is a highly responsible professional position under supervision of the Director of Nursing.  Position will work in Healthy Start Program for Columbia and Hamilton counties and provide care coordination services by assessing client's health status and unmet needs, participates in public health clinics; and performs related duties as required.  Interacts with a variety of health care providers and community organizations.


Provides case management/care coordination services to Healthy Start participants.  Participants include pregnant women, infants/children, interconceptional women and their families as defined in the Healthy Start Standards and Guidelines.  Services include assessment of risk factors/needs; determination of client ability to manage risks/needs; providing referrals to community resources; follow-up for access to resources; provision of educational services including childbirth education, parenting, interconceptional education, smoking cessation counseling, and breastfeeding support/education.  Collaborates with the Healthy Start team to develop and maintain an Individual Plan of Care that is appropriate to the need of the client and in accordance with the Healthy Start Standards and Guidelines.  Services are provided in the client home, clinic office, or in a community setting.


Provides community outreach education and activities that can positively effect health outcomes, assists in health fair activities, provides documentation of such activities; consults with, and/or provides technical assistance to public and private entities concerning health care.


Employee may be required to work before, during or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the employee includes working in special needs or Red Cross shelters or perform other emergency duties including but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.


Must be licensed as a Registered Nurse in the state of Florida. Experience managing client care or social services preferred.  Experience documenting client information in electronic health systems preferred.  Experience making home visits to interface with clients preferred.  Must have a valid driver’s license.  Experience working with pregnant women and/or children preferred.  CHD experience preferred.

Duties and Responsibilities


This is a professional position assigned to Columbia and Hamilton County Health Departments and reports to the Nursing Director.  The responsibilities include but are not limited to case management for Healthy Start Clients.  Provides services to pregnant mothers and infants to improve birth outcomes and infant health.  Conducts assessments to determine risk and educational levels.  Assesses the need for services.  Assist with obtaining available services through referrals.  Provides information and education on parenting, childbirth, breastfeeding, family planning, nutrition, and smoking cessation.  Conducts regular home visits with parents according to risk levels.  Attends meetings, and participates on conference calls to determine better ways to serve clients and improve county rates regarding teen pregnancy, infant mortality, preconception and interconceptional health of women.  Will coordinate educational needs of the Healthy Start Clients in Columbia and Hamilton County.  Works to improve pregnancy outcomes for at risk Healthy Start Clients.  Other duties include Quarterly Reporting of Healthy Start Services, QA/QI, and compliance with Healthy Start Standards and Guidelines.


Responsible for duties to include but are not limited to performing Care Coordination Services for Healthy Start clients, completes chart audits and prepares and participates in annual program review with Healthy Start Coalition staff, participates in community outreach for marketing Health Start Program and services.  Will participate in client satisfaction surveys and quality assurance programs as related to the Healthy Start Guidelines and Standards.


Provides case management services initially and on an on-going basis for risk assessments, developmental screening, and psychosocial evaluation to high risk pregnant women or infants through the Healthy Start Program.  This includes but is not limited to initial contacts, initial assessments, home visits, outreach, developing and fostering community partnerships while maintaining high visibility within the community.


Assesses the educational needs of Healthy Start Clients who are at risk for poor pregnancy outcomes.  Providing educational classes to meet educational needs identified through assessments for or from clients requests.  Assisting with referrals to meet educational needs.


Design and develops partnerships with clients, and providers to meet the needs of the Healthy Start Clients.  This would include but is not limited to the following:


A. Provide prenatal skill building involvement and support of the Health Start Clients.


B. Provide tobacco cessation education utilizing current curriculum approved by the Healthy Start Coalition.


C. Provide childbirth education with current curriculum and maintains certification through scheduled training updates.


D. Provides parenting classes with current Healthy Start Coalition approved curriculum.


Responsible for securing the designated information sets for the purposes of protecting confidentiality, data integrity, and appropriate access, for all information, both confidential and public record, which is stored in hard copy or electronic formats.
