The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, Down syndrome, children age 3-5 at high risk of developmental disabilities, cerebral palsy, Prader-Willi syndrome, Phelan-McDermid syndrome, and intellectual disabilities. There is no charge or co-payment for services provided through the Agency.

BACKGROUND SCREENING REQUIREMENT:

It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.

***DUTIES:

This is a professional position responsible for Medical Case Management and consultation for individuals who are eligible for services through the Agency. The incumbent works in an interdisciplinary and multidisciplinary fashion within the construct of the Regional structure. Primary duties include, but are not limited to: clinical oversight of medical case reviews; monitoring of activities of medication administration of providers trained under 65G-7; ICF/DD responsibilities including Utilization Control and choice counseling; conducting on-site reviews of service delivery to ensure compliance with federal and state statute, rule and procedure; assisting with quality assurance activities to improve services for recipients of the Agency; and providing consultation and technical assistance regarding medical services. This position works collaboratively with state office medical/nursing team. Specific duties include:

Serves as Medical Case Manager to monitor Medication Administration Rule FAC 65G-7 within the Northeast Region (Daytona Beach Office area). Services include but are not limited to coordination of approved RN trainers, monitoring compliance in licensed group/foster homes, technical assistance and training for support coordinators, supported living coaches, all providers and program office staff.

Conducts nursing assessments and develop written reports for the purposes of assisting with treatment team decisions regarding placement, emergency and special intervention, court orders, problem solving and eligibility determination. 

Serves as coordinator for individuals’ health related supports for service reviews, follow-up on reasons for illness, injury or accident as requested. Provides clinical oversight of medical case reviews as needed and requested.

Coordinates and/or performs all Agency required ICF/DD facility duties to verify level of care, all 180 and 360-day Continued Stay Reviews in the counties in the Northeast Region (Daytona Beach Office area) ICF/DD facilities. Assures ICF/DD database is maintained to track all activities required. Assign level of care for consumers seeking Medicaid eligibility for inclusion in the Medicaid Waivers. Utilization Control and Choice Counseling for individuals residing in ICF/DD facilities within the Northeast Region (Daytona Beach Office area).

Attends and actively participates in regularly scheduled meetings to improve service integration, including but not limited to CMAT meetings, staff meetings, MCM reviews, reports of death, consumer specific meetings and staffing, special reviews and others as assigned.

Provides training and consultation relative to medical land health related issues.  Consults with families, vendors, service providers, consumers and health professionals. 

Provides any needed follow-up for Level II PASRRs

Performs other duties as assigned.
