The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, Down syndrome, children age 3-5 at high risk of developmental disabilities, cerebral palsy, Prader-Willi syndrome, Phelan-McDermid syndrome, and intellectual disabilities. There is no charge or co-payment for services provided through the Agency.

DIRECT DEPOSIT PROGRAM - As a condition of employment, a person appointed to a position in State government is required to participate in the Direct Deposit Program.  Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. BACKGROUND SCREENING REQUIREMENT:

It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.
No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.
Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement, and Federal criminal records checks through the Federal Bureau of Investigation, and may include local criminal records checks through local law enforcement agencies.

REQUIREMENTS:

Must have experience with Microsoft software programs (Outlook, Word, Excel, Access, and PowerPoint).

 

Must have experience working directly with persons with developmental disabilities.

 

Must have experience working with criminal defendants as either a case manager or in direct care setting. 

 

Position may require some travel to surrounding areas, applicant must be willing and able to travel for work purposes.

***DUTIES:

 

Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service. This includes, but is not limited to, always acting in a responsive, professional, courteous manner with the employees, customers and public we serve at all times.

 

The person in this position will provide case management of forensic clients to ensure they are properly placed, getting proper services, and are not putting themselves or the community at risk.  This person will specialize with this population and will be the primary liaison as it relates to F.S. 393.11 commitments. They will also assist with those clients committed under F.S. 916 and in secure facilities to ensure when they are released from commitment, they have can access services.  They will also complete Eligibility Determination for forensic and other assigned applicants.  Specific duties include:

Liaison between facilities and region for clients committed under FS 916 or FS 393.11, including but not limited to assisting with eligibility issues, discharge planning, and linking clients to services. 

Ensure clients committed by courts under F.S. 393.11 have adequate community services to ensure they meet court requirements.

Liaison with court/probation staff to ensure client fully understands expectations.

Regular contact with forensic clients committed by courts under F.S. 393.11 and their service providers to ensure adequate services are in place to keep client and community safe.

Monitor and ensure reporting requirements are met.  This includes arranging annual 393.11 evaluations and ensuring they are completed and filed on time, and ensuring the evaluator has all the information needed to appropriately evaluate the client.

Assist regional court staff in identifying psychologist to conduct evaluations, providing technical assistance in the elements of the law that need to be addressed in the report. 

Provide technical assistance and training to waiver support coordinators and other service providers working with forensic clients, while assisting with provider development to serve this group.

Reviews applications for APD and iBudget waiver services to determine eligibility.  Ensures compliance with mandated timeframes for eligibility determination. Prepares and sends Notices of Eligibility and Ineligibility.  Ensures documents are stored as required and maintains appropriate tracking of the status of applications.

 

Participates in Division of Administrative Hearings (DOAH) hearings related to appeals of eligibility determinations as assigned.

 

Other duties as needed.
