The Department of Health in Pinellas has a Family Support worker opening in the Medical Services Program based at the Mid County Location. The successful applicant will work in the Primary Care Clinic. 

Job Summary-Family Support Worker: The Family Support Worker will work as a Community Health Advocate (CHA) assisting clients within the Pinellas County Health Program for primary care services. The incumbent will serve as patient advocate by coordinating access to available services.


The successful incumbent will perform the following duties and must be professionally competent to perform in any multiple clinics and must can deal effectively with customers of various types of backgrounds and cultures:

  • Serve as patient advocate by coordinating access to available services
  • Ensure newly enrolled clients receive appointments within the established timeframes
  • Coordinate patient specialty care referrals, which includes mailing referral letters and answering questions regarding the status of specialty care referrals
  • Ensure proper information flows from the primary care home to referral sites
  • Work with clients to make sure they keep appointments, schedule appointments 
  • Link patients to social services such as transportation, economic and food resources
  • Perform weekly and monthly record keeping and reporting of data and statistics related to those served which meet the requirements
  • Assist in the preparation of reports
  • Attends and participates in staff meetings, in-service training, and other professional and staff development activities

Minimum Qualifications:

  • One year experience using an electronic health record
  • Florida driver’s license and access to dependable transportation or able to obtain within 30 days of hire
  • Ability to work with persons of a different race, language, culture, or socioeconomic status. 
  • At least one year experience working in a medical setting

Desired Qualifications:

  • Knowledge of Medical Terminology
  • One year experience working with Excel

  Other Related Duties as Assigned:

  • Performs other duties as required
  • Keeps supervisor informed
  • Attends staff meetings as required
