Annual Salary:  $34,820.24 - $46,718.36

Paid Bi-Weekly

MINIMUM QUALIFICATIONS

  • Licensure as a Registered Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27 
  • MDS Coordinator experience required 
  • Valid CPR card
  • A Bachelor's degree from an accredited college or university with a major in nursing or a related field and two (2) years of nursing experience.  A master's degree from an accredited college or university in nursing, nursing education, public health, or related filed may substitute for one (1) year of the nursing experience. Professional nursing experience may be substituted on a year-for-year basis for the required education
  • Basic or Proficient computer skills working with MS Outlook, Word and Excel
  • Must participate in Direct Deposit
  • Successful Completion of a Level II background screening and drug test

 

PREFERRED QUALIFICATIONS:

  • Prefer at least one (1) year experience working in a long-term care facility with a geriatric population
  • MDS Certification
  • Experience with MDS data entry and RAI assessments.

 

POSITION DESCRIPTION:

  • Schedule or oversee scheduling of all resident MDS assessments to assure timeliness. Coordinate and oversee all functions of the MDS assessment process to assure compliance with Federal and State requirements.  Participate in resident level of care determination upon admission. Participate in tracking changes in residents’ level of care to assure timely scheduling of MDS assessments.  Be familiar with residents’ condition and care needs.
  • Review all MDS assessments and related forms to certify timely completion by signing the MDS forms on the appropriate sections.  Review reports of timely completion of residents’ MDS assessments and care plans and take follow-up action as necessary. Participate in the Quality Assessment and Assurance Program, in cooperation with the Quality Assurance Coordinator. Analyze Quality Indicator reports for potential resident concerns and problems and review residents’ care plans to be sure this information is included.
  • Conduct and/or participate in interdisciplinary team and rehabilitation conference meetings.  Maintain confidentiality of resident and facility information.  Complete reports and assignments accurately and adhere to established time schedules.  Establish open lines of communication between all departments. Observe infection control procedures relating to Nursing services.
  • Consistently work cooperatively with administration, Nursing Service Personnel, Physicians, community agencies, residents, families, and Consultants.  Perform other duties as assigned by the Director of Nursing Service. Perform all duties assigned in a cooperative, effective, timely, and professional manner.  Be responsible for the safety of residents under his/her supervision.
  • Perform other related duties and projects assigned.  Maintain current certification in Basic Life Support and perform this skill when necessary.
