JOB DESCRIPTION

 

This individual will be responsible for all supervisory duties including, but not limited to, hiring and selection, developing performance plans, conducting performance appraisals, planning, and directing their work, approving leave, and taking disciplinary action as appropriate.  This is a professional nursing position responsible for Specialty Clinic Nursing and Nursing Care Coordination for children with severe and chronic illnesses. The professional occupying this position shall be assigned to specific clinic specialty areas to provide assistance to the physicians in the clinic to offer support and teaching to parents; to assure that each child receives optimal care. The professional may also be assigned to assist clients with psychosocial needs.

 

This position reports to the Nursing Director. Specialty Clinic activities are targeted to champion excellence in the delivery of health care for children with special health care needs. This professional exhibits effective communication, displays courtesy, respect and understanding when dealing with family members and patients, the public, coworkers and staff of other community agencies.

 

Responsibilities include but not limited to:

 

  • Conduct care coordination assessments for children with special health care needs.
  • Develop a comprehensive patient-focused care plan that shows collaboration between the child’s provider team, the child and family, and other community resources. 
  • Coordinate and document services for the children/families, including multi-program/multi-disciplinary meetings as indicated.
  • Follow-up with and provide supportive counseling and guidance to patients and families.
  • Assess community resources, identify gaps in service and report these gaps to the Nursing Director.
  • Utilize all resources available to address the complex medical issues of the patient.
  • Effectively collaborate with the healthcare delivery team using a multi-disciplinary approach.

     

    Specific Clinic Duties and Responsibilities:

     

  • Perform the Pre-Clinic review process, which includes diagnostic testing and reports in the required timeframe.
  • Perform the clinic day check-in counseling and assessment process.
  • Perform the Post-Clinic counseling process.
  • Perform the Post-Clinic record review.
  • Able to identify/assess an emergent patient situation and implement the appropriate resources.

     

    Document all patient activities in the current CMS approved electronic documentation application according to the established CMS and documentation standards to include the following:

     

  • Assessments
  • Care Plans
  • Care Coordination Activities
  • Eligibility screenings
  • Redeterminations
  • All other pertinent patient information
  • HEDIS Performance Measures as appropriate
  • Utilizes guidelines as outlined in the CMS Requirements for Care Coordinators while caring for the enrollees receiving Private Duty Nursing Services to include the following:
    • Maintain monthly (or more frequently if needed) contact with the patient and/family member
    • Must participate in the Multidisciplinary Team Meetings that occur at least every 6 months

       

      Provide education for the family and patient to include the following:

       

  • Prepare the parent for procedures and /or treatment modalities
  • Educate the parent on child development physiologically, psychologically, and socially
  • Review physician assessments and treatment plans
  • In conjunction with the child’s physician and social worker evaluate the potential need for protective action in the case of an abused/neglected child.
  • Collaborate with Social Worker and Supervisor and prepare to make a referral to the appropriate agency for follow up
  • Performs other related duties as required, so that the total needs of the child and family are met

     

    Ability to utilize the following systems:

     

  • CMS/Kids/KidCare
  • Ped-I-Care resources and collaborate with the Provider Liaison when necessary
  • Have the ability to trouble shoot computer issues as necessary and know how to use resources like ‘Help Desk’ tickets
  • View and/or schedule children for clinic appointments in the MED3000 ~ PM/CC system 
  • Proficient with Electronic Health Record (EHR), Windows, and Outlook Email System

     

    Knowledge, skills and abilities, including utilization of equipment, required for the position:

  • Knowledge of nursing principles, practices and techniques, with pediatric nursing experienced preferred.
  • Knowledge of human anatomy and physiology, and pathophysiology.
  • Knowledge of disease management.
  • Ability to manage multiple projects simultaneously.
  • Ability to supervise people.
  • Ability to determine work priorities, assign work and ensure proper completion of work assignments.
  • Ability to provide emergency nursing services to pediatric patients by activating the Emergency Management System and initiating CPR until help arrives.
  • Ability to participate in the planning and implementing of treatment for patients.
  • Ability to work with patients and family members that have a variety of physical and/or emotional challenges.
  • Ability to plan, organize and coordinate work assignments.
  • Ability to communicate effectively among staff, public and with families.
  • Ability to establish and maintain effective working relationships with other members of the health care team.
  • Ability to effectively interview and assess the health care of clients. 
  • Ability to analyze complex reports and determine the service utilization habits of clients and to evaluate the appropriateness of that utilization.
  • Able to use the AED in the event of an emergency.
  • Maintains a positive, upbeat, productive attitude toward the mission, vision, values and leadership of CMS
  • Will be supportive of Head Quarters’ projects and project implementations and initiatives by being open to and embracing change.
  • Follow Protected Health Information (PIH) and HIPAA guidelines.
  • Be able to determine whether emails must be encrypted and know how to encrypt an email message.

     

    Licensure/registration/certification requirements:

  • Licensure as a Registered Professional Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27 and three years of professional nursing, psychiatric nursing or nursing instruction experience. 
  • A bachelor's degree from an accredited college or university in nursing or nursing education can substitute for one year of the required experience.
  • A master's degree from an accredited college or university in nursing, psychiatric nursing or nursing education can substitute for two years of the required experience.
