The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, Down syndrome, children age 3-5 at high risk of developmental disabilities, cerebral palsy, Prader-Willi syndrome, Phelan-McDermid syndrome, and intellectual disabilities. There is no charge or co-payment for services provided through the Agency.

DIRECT DEPOSIT PROGRAM - As a condition of employment, a person appointed to a position in State government is required to participate in the Direct Deposit Program.  Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. BACKGROUND SCREENING REQUIREMENT:

It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.
No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.

***DUTIES:

Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service. This includes, but is not limited to, always acting in a responsive, professional, courteous manner with the employees, customers and public we serve at all times.

This is a professional position responsible for independently performing a variety of activities related to APD consumers receiving Supported Living Coaching and community base Companion and Personal Support Services as covered by the Medicaid HCBS Waiver and APD General Revenue funding. Also, responsible for the development and on-going training of the Supported Living Provider Network consisting of appropriately licensed providers. The incumbent provides technical advisement to the provider network in their day-to-day operations as well as assists in care planning and problem-solving. Facilitates quarterly supported living meetings and maintains data for performance indicators for the assigned caseload as well as tracking and trending of pertinent incidents and issues. Processes and approves/denies all requests for supported living start-up funds and emergency stipends. Frequently utilizes state vehicle or personal vehicle to travel within the assigned area to personally visit consumers in their homes and facilitates quarterly supported living meetings. Must maintain a valid Drivers’ License and vehicle insurance.

Provides or arranges for Targeted Case Management for known-risk supported living consumers and develops targeted interventions and trainings to optimize provider care and service to those consumers. Also develops standard and ad-hoc training for new and legacy Supported Living service providers as well as provides technical assistance/monitoring for supported living services. The incumbent will collaborate with Waiver Support Coordinators and consumers that are seeking guidance on specific program specifics on interpretation of Medicaid HCBS Waiver Handbook language related to Supported Living Supports and Services. Promotes and develops compliant supported living arrangements with providers and follows-up on those arrangements.

Works closely and collaboratively with the QA/QI Workstream and tracks and reviews Incident Reports related to Supported Living Services and Delmarva provider monitoring reports and addresses findings through technical assistance/remediation and develop aspects of a Corrective Action Plan (CAP) as well as follows-up through resolution.  Requests plan of remediation as needed, obtains documented evidence of provider corrections to 100% of the review findings. Collaborates on quality assurance, performance improvement and enhancement of provider service delivery practices, training and education, customer service, investigations and recoupment of funds, and networking, communicating and collaborating with others while upholding HIPAA requirements.  The incumbent's duties include, but not limited to, the following:

As it relates to Supported Living tracks, investigates, addresses, and documents any service delivery grievances/complaints, suspicions of misuse of funds, fraud, programmatic violations, incident reports, protective services investigations, and follows-up through with the QA/QI program.  Identifies, calculates, negotiates, consults on and ensures completion of a Plan of Remediation and obtains documented evidence of provider corrections and improvements.  Makes recommendations for further action on the provider status as needed.

Requests and compiles Supported Living provider records and conducts audits as needed.  Writes findings summary with recommendations and identifies recoupment and performance patterns/trends.  Addresses findings with provider at technical assistance/remediation, follows-up with provider for Plan of Remediation completion and obtains documented evidence of corrections/improvements to 100% of the findings. Uses ABC, FMMIS, and iBudget proficiently. Follows Agency’s protocols toward resolution and full implementation of corrections/improvements.

Participates in meetings, conference calls, training, workshops/workgroups, and other miscellaneous activities as needed.  This includes Delmarva quarterly meetings, Region’s Quality Management meetings, and Steering Committee meetings. Works on special projects and other assignments through to completion and meeting deadlines; performs other duties as assigned.
