THIS IS AN OPS POSITION

The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, Down syndrome, children age 3-5 at high risk of developmental disabilities, cerebral palsy, Prader-Willi syndrome, Phelan-McDermid syndrome, and intellectual disabilities. There is no charge or co-payment for services provided through the Agency.

BACKGROUND SCREENING REQUIREMENT:

It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.
No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.

REQUIREMENTS:

Must have experience working with individuals with developmental disabilities.

 

Must have a valid driver’s license.

***DUTIES:

 

Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service. This includes, but is not limited to, always acting in a responsive, professional, courteous manner with the employees, customers and public we serve at all times.

 

This is a professional Other Personal Services (OPS) position with the responsibility of conducting evidence - based assessments, for the Agency for Persons with Disabilities using the Questionnaire for Situational information (QSI) for all individuals enrolled on the iBudget waiver and Consumer Directed Plus (CDC+) waivers as well as individuals on the APD waitlist. This position reports to the Clinical Supports Regional Program Supervisor.  

 

Successfully completes a delegated minimum of needs assessments per month (successful is defined as reliable / valid score as evaluated by area master trainers), which includes scheduling a face to face interview with the individual with a developmental disability, the individual's guardian, and family.  Interviews with the individual's caregivers, health care personnel, support coordinator or service providers will be conducted, as appropriate.  Completes the Questionnaire for Situational Information (QSI) and maintains inter-rater reliability.

 

Provides individuals, family members, and provider information/literature on available resources in the community that might be available based on the needs identified during the meetings held and QSI assessments completed.

 

Reviews the individual's central record including recent assessments, progress notes from medical records, school records, previous support plans, and relevant information from other collateral sources, as appropriate.

 

Schedules meetings or telephonic sessions with the individual eligible for services from APD, family members, guardians, Waiver Support Coordinators, other providers and support staff to complete a comprehensive review of needs for completion of the QSI assessment.

 

Communicates needs for follow-up by the Medical Case Manager or Regional Behavior Analyst following completion of QSI assessment.

 

Report needed updates in Iconnect to assure the support coordinator maintains accurate and current information.

 

Successfully completes training and achieves inter-rater reliability on the Questionnaire for Situational Information (QSI) Assessment tool.  Performs other duties as assigned.
