MINIMUM QUALIFICATIONS:‎
Bachelors degree in Social Work (BSW).‎
One (1) year experience in a health care setting as a social worker.‎
Successful completion of our employment screening process to include, but not limited to, drug test and finger ‎printing.‎

 

PREFERRED QUALIFICATIONS:‎
Two (2) or more years experience in a health care setting.‎
Experience working in a Nursing Home.‎

 

POSITION DESCRIPTION:‎
Serves on Care Plan team with the responsibility of insuring that the resident treatment objectives are consistent ‎with his/her overall treatment process.‎
Maintain progress notes on each resident as needed.‎
Provides social and behavioral expertise in the development of treatment plans and management of social ‎services for residents and/or families with psychosocial needs resulting from complex medical problems.‎
Consults with doctor, nurses and other professionals regarding emotional and behavioral problems encountered in ‎the treatment process.‎
Administers evaluation assessment, through interview, to determine individual therapeutic program needs and ‎interests.‎
Use such findings to develop, organize and implement innovative therapeutic programs to meet the mental ‎health needs of the individual resident.‎
Interviews residents and relatives regarding admission procedures and release plans.‎
