MINIMUM QUALIFICATIONS:
- Current DOH Employee.
- At least 3 years of professional experience providing services, health education, community outreach, or case/program coordination to maternal and child health populations OR a bachelor’s degree from an accredited college or university in public health, health promotion, community health education, health administration or a related field.
- Verifiable experience planning and delivering health education presentations or outreach sessions to community groups or clients.
- Verifiable experience creating, building, and maintain partnerships with community organizations, healthcare providers, or community base organizations.
- Incumbent must have the physical ability to kneel, bend, lift and carry up to 15 pounds, and maneuver different types of terrain, and use fine motor skills to do job duties and responsibilities.
- Florida Driver’s License and access to a personal vehicle to perform job duties and responsibilities of this job.
- Incumbent must be flexible to work beyond the standard 8:00am-5:00pm hours to support participation in community meetings, health fairs, workshops, and other outreach activities. The incumbent must be available to work occasional evenings and weekends as needed.

JOB DUTIES AND RESPONSIBILITIES:
This position, the IMPACC Health Promotion Specialist, will lead the "Sister Friends" initiative under the Catalyst for Infant Health HRSA grant. This role focuses on enhancing maternal and infant health outcomes by providing education, resources, and support to families and mothers. Responsibilities include conducting outreach events, leading educational classes like infant CPR, facilitating closed-loop referrals to healthcare programs like WIC and SOBRA Medicaid, and connecting families to community services that best meet their needs. The coordinator will also represent the organization at community events and collaborate with partners to address health outcomes and improve access to care. This position requires a flexible schedule to meet with mothers and families at times that are most convenient for them. This position will also support the Maternal and Fetal Infant Mortality Review (FIMR) project in collaboration with the Florida Department of Health-Orange County and Healthy Start Coalition of County. The aim of the Maternal and FIMR is to enhance the health and well-being of women, infants, and families by improving the community resources and service delivery systems available for families.
Project Management: Oversee the implementation of the Sister Friends Project in alignment with the Florida Healthy Babies (FHB) Initiative. Lead prenatal and postpartum support groups and educational sessions for families. Utilize the big sister/little sister model to foster community support among mothers. Collaborate with community-based organizations to provide comprehensive support services.
Close Loop Referral: Identify at-risk families through outreach and assessments. Facilitate referrals to healthcare programs such as WIC and SOBRA Medicaid. Ensure referrals are completed by following up with clients and service providers. Maintain open communication to confirm that families successfully access the referred services.
Community Outreach and Representation: Plan and participate in outreach events within the community. Represent the organization at events hosted by community partners and stakeholders. Build and maintain relationships with local organizations and hospital programs, providing technical assistance and support for initiatives like the IMPACC programing.
Resource Connection & Client Engagement: Stay informed about available community services and resources. Connect families to services that best meet their unique needs. Build and maintain relationships with community organizations and service providers. Adjust work hours to meet with mothers and families at times most convenient for them, including evenings and weekends as needed. Provide one-on-one support and guidance to clients, fostering trust and engagement.
Stakeholder Partnership: Attends and participates in scheduled HRSA, related meetings, and community events, as identified to provide information and updates. Completes additional reports and data gathering for statistical analysis as required by funders and stakeholders.

KNOWLEDGE, SKILLS AND ABILITIES, INCLUDING UTILIZATION OF EQUIPMENT, REQUIRED FOR THE POSITION:
- Knowledge of maternal and child health programs and resources.
- Experience in community outreach, program coordination, or health education.
- Excellent communication skills, sensitivity to needs and experiences of pregnancy, infant loss, and post-partum families and cultural competency.
- Knowledge of working with community agencies and those who live in the community.
- Experience conducting presentations and leading group sessions.
- Ability to work independently, attention to detail, with a flexible schedule, including some evenings and weekends.
- Ability to accomplish tasks in short time frames.
- Ability to kneel, bend, lift and carry up to 15 pounds, and maneuver different types of terrain, and use fine motor skills daily.
- Computer skills (ability to use applications such as spreadsheets, database, and analytical software).
- Effective verbal and written communication skills.
