Your Specific Responsibilities:
This position provides case management services for the Housing Opportunities for Persons with AIDS (HOPWA) and Tenant Based Rental Assistance (TBRA) programs in Area 14 (Hardee, Highlands, Polk counties). Case management activities provided through office and field visits.
Duties including but not limited conducting eligibility determination for HOPWA and TBRA.; Development of housing needs assessment, plan of care and budget assessment; client contact follow up and care plan implementation; Dwelling inspections to verify housing suitability and safety issues. Assists in resolving disputes between client, landlord, mortgage, or utility companies to effectuate stable housing situations. Provides referral and linkage to housing support services.
In instances of extended absences, vacancies or need for general coverage, position will serve as a back-up to medical case managers assisting with job functions including but not limited to eligibility determinations, completion of needs assessments, plans of care, client linkage and referral to necessary services. All services provided through office and /or field visits.
HOPWA Case Management
Interview, Eligibility Determination/Re-Determination
Upon receiving referral for housing assistance, conducts interview to determine eligibility for assistance according to HOPWA guidelines. Interview and assessment due within seven business days of request for services.
Housing Needs Assessment
Face-to-face assessment addressing the following areas but not limited to housing history; actions, behavior or circumstances led to housing problems; barriers, strength and supports client has and needs to enter and remain in stable housing; medical and treatment adherence; mental health, substance use, financial and legal concerns. Assessment is due at the completion of the interview and eligibility determination. Completed needs assessment is scanned into CAREWare.
Housing plan of care development (HOPWA/TBRA)
Findings from the needs assessment must flow into the plan of care and be addressed in the notes. Plan of care addresses goals, objectives and barriers. In addition, the Housing Plan of Care shall address the following financial aspects:
Assisting the client to plan and budget their finances; Accessing additional income sources and social services; Time frames for completing various disability applications, contingency plan in the event the disability application is denied. POC is due at the completion of the interview and eligibility determination. Completed plan of care is scanned into CAREWare.
Plan of care implementation and client contact
Responsible for the implementation and oversight of the care plan. Completion of tasks listed in the care plan. Documentation shows need for services are being addressed in an ongoing manner. Re-evaluation on a monthly basis for six months with adaptations as necessary. Monitoring plan to assess the implementation and efficacy of the plan. Refer clients to additional support services that will remove barriers to obtaining and maintain permanent housing
Completes and maintains accurate documentation of all client specific activities (HOPWA; TBRA; RW Case management)
Documentation including direct client interactions and roles undertaken on behalf of a client. Documentation includes written case notes and required form scanning.
Detailed case notes how client’s housing needs are being addressed, how barriers to meeting these needs are being overcome, and how client is progressing towards POC goals. Case notes, form scans and all required information on clients are entered in the CAREWare and HMS databases within three business of client interaction. Documentation on inactive and closed cases are completed, thoroughly exploring all methods of client contact available and allowed, referral to community resources where applicable, and follow up with all providers of record.
Resource Identification
Identify and Develop collaborative resources with community and other stakeholders to facilitate the accomplishment of goals of clients enrolled for housing service. Conduct field visits and outreach activities to establish said relationships. discussing not only the HOPWA program and services, but also issues of interest and concern of the program. Maintains documentation of field visits and outreaches. Prior approval needed for field visits and outreaches. Submits program progress reports as requested.
Housing Administrative
Maintains documentation of all program related field visits, dwelling inspections, community partner visits, engagement and outreach activities. All visits and activities must be prior approved. Generates monthly program reports as required
If required, conduct visits to all properties prior to client moving into the property to insure it is a safe and habitable environment. Make monthly calls and complete annual assessments and annual HUD inspections for all Shelter Plus Care clients receiving assistance. Contact landlords, owners and utility providers to verify acceptance of 3rd party payments. Prepare invoices for payments such as: utilities, mortgages and rental assistances. Submit invoices for rental payments. Submit utility invoices and copy of Rental Payments. Collaborate with the third-party payor source to ensure payment is sent on behalf of client. Complete spending logs and data collection logs for all services and payments made on behalf of the client.
Agency Administrative
Completes EARS/CSRS and timesheets within required timeframes. Submits within required timeframes but not limited to: cell phone verifications, travel documentation, invoices, monthly reports etc.
Maintains client confidentiality and security in accordance DOHP50-10-10 Information Security & Privacy Policy Confidential Information.
Required Knowledge, Skills, and Abilities:
- Knowledge of theories and practices in counseling and social work.
- Knowledge of interviewing techniques.
- Ability to provide counseling and social work services to others.
- Ability to plan, organize and coordinate work assignments.

Qualifications:
Preferred
- A bachelor’s degree in psychology, social work, or human services.
- A bachelor’s degree in any field of study with at least six months of case management experience.
- Applicable experience, which may be substituted on a year-for-year basis for the required education.
