Your Specific Responsibilities:

  • This position is responsible for developing and implementing activities associated with surveillance and investigation of suspected and confirmed cases of HIV/AIDS.  This position functions under the technical supervision of the Program Supervisor.  This position requires that the individual possess(es) a knowledge of health care services, principles and methods of Epidemiology, and the ability to collect, compile, and analyze data from surveillance activities.
  • Advises and assists county public health units in planning, coordinating, and evaluating program activities related to HIV/AIDS Surveillance.
  • Acts as security information custodian for the HIV/AIDS surveillance program, in accordance with Department of Health and Polk County Health Department Security Policy 3.
  • Establishes active liaison(s) with medical institutions and medical specialists most likely to diagnose and provide treatment for AIDS patients.  Initiates routine procedures in conjunction with CPHU personnel and determines if reported cases meet case definition.  Maintains HIV/AIDS reporting system (HARS) for Polk, Hardee and Highlands counties.
  • Provides technical assistance, consultation, and training to state, city, or local health officials, private and public organizations and institutions, and other groups regarding investigation and surveillance of AIDS.  Develops close working relationships with all groups associated with HIV/AIDS, including organizations for groups at increased risk of HIV/AIDS.
  • Directs data collected activities for assigned geographical area of the state and insures reporting of HIV/AIDS cases to the central office in Tallahassee.
  • Compiles a quarterly HIV/AIDS surveillance report which includes cumulative HIV and AIDS morbidity and mortality data for Area 14.
  • Assists in the development of methods to expand the system for HIV/AIDS surveillance in order to insure 100% reporting of all cases of HIV/AIDS.
  • Assists in the development of various special studies of HIV/AIDS in selected populations in Florida, to include coordination of date collection activities, compilation of data collection activities, compilation of data, and analysis.
  • Attends workshops, conferences, and committee meetings relating to HIV/AIDS.
  • Conducts epidemiological investigations of all confirmed HIV/ AIDS cases which are not members of known risk groups and aiding in general field investigations of cases or suspected cases of HIV/AIDS as designated by the central office in Tallahassee.
  • Data Collection, quality assurance, quality improvement and contract management of HIV/AIDS clinic service funding grants.

 

Ryan White Program Management Activities

  • Assists the Health Center Administrator in managing the Ryan White Program for Area 14 through: Reporting; Program Monitoring; Data Collection and Analysis; and Clinical Quality Management.
  • Reporting:  Incumbent will be responsible for the accurate and timely reporting that is required from the Ryan White Program for local, state, and federal reports as required.  This includes but is not limited to: monthly, local statistical reports; monthly Part B reports required by the state of Florida; annual, semi-annual or other required reports for HRSA or other federal agencies.
  • Program Monitoring: Incumbent shall monitor compliance with all HRSA and state guidelines regarding the provision of care and case management services for our eligible clients.  This monitoring will take place through the regular review of patient medical and case management records currently found in HMS and CareWare but will not be limited to those databases.  Monitoring will include monthly reports to the Health Center Administrator and regular discussion with Clinical RN supervisor, Case Management Supervisor, medical providers, and others as necessary.
  • Data Collection: Incumbent will lead the development of data collection activities relating to all aspects of the Ryan White Program in Area 14. Manages the creation and maintenance of database(s) for the Ryan White Program, and the manages the creation of user-friendly data reports. Data is currently found in HMS, CareWare, FL SHOTS, and our local Access Database.
  • Clinical Quality Management: Incumbent will serve as the back up to the Health Center Administrator for the Clinical Management Team and will assist the HCA in running the meetings, recording information, assisting units within the Ryan White Program. CQM is a requirement of the Ryan White Program and forms the basis from which training and enrichment opportunities are developed for Ryan White Program Staff.  Incumbent will also serve as an advisor to the HCA in planning and implementing regular staff trainings.

  • Performs other related duties as assigned, including performance of additional interpreter duties, consistent with skill level, and as needed.

Qualifications:

  • Valid Florida Driver’s license
  • Preferred: Bachelor’s degree from an accredited college or university

Emergency Duty: Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
