Duties and Responsibilities

This is a professional nurse position working in the treatment of patients in a clinic setting for both Holmes and Washington CHDs.

Applies skilled nursing principles and techniques to uncover health problems through observation, interviews, technical nursing skills and record analysis.  Offer professional nursing recommendations to clients and serves as a liaison for other agencies through coordination and referral for care and treatment.

Interviews patients and records complete history.  Works clinic at Holmes/Washington CHDs as assigned.

Counsels patients and explains the effectiveness, advantages, and disadvantages and side effects to available methods of birth control, immunization program, communicable disease control, Tuberculosis (TB) control and Sexually Transmitted Disease (STD) control.

Provides individual or group counseling and teaching as needed.

Performs diagnostic and/or screening test as authorized and obtain laboratory specimens when indicated.

Assists the physician/APRN in rendering medical services and providing the patient with medication/procedure under medical direction.

Educate patient/family about basic health principles and importance of preventive measures.

Maintain appropriate health records for program entry.

Serves as backup for STD program reporting for both CHDs.

Serves as backup for Epidemiology and Tuberculosis investigation/reporting for Holmes/Washington CHDs.

Serves as backup for Florida AIDS Drug Assistance Program (ADAP) program for both CHDs.

Attends workshops, meetings, and in-services for professional growth.

Accepts other work assignments as delegated by supervisor or Executive Community Health Nursing Director.

Serves as back-up Information Custodian for Epidemiology (EPI) and backup Information Custodian for Pharmacy, HIV/AIDS, STD, TB, and Clinic FAX.  Responsible for:  maintaining and assuring confidentiality, serving as security resource within his or her division and as a member of the security team, creating acceptable protocols for the handling and data flow of information stored in their designated information set, meeting with other information custodians and security coordinator as needed, report security-related issues to the Security coordinator, and assist with the annual security risk assessment.  Responsibilities include maintaining information in accordance with state and federal statutes, rules regulations and Department of Health Policies, Protocols, and Procedures.

Performs other duties and responsibilities as assigned by supervisor.
