Your Specific Responsibilities:

The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Manatee County in its mission to protect, promote and improve the health of all people in Florida through integrated state, county and community efforts and its vision to be the Healthiest State in the Nation.  Furthermore, the incumbent is expected to incorporate the agency values of Innovation, Collaboration, Accountability, Responsiveness and Excellence in all aspects of their work.

 

The incumbent works within the community to expand the education and testing program to target populations for HIV/AIDS.  He/she works closely with various community organizations/schools to help increase HIV awareness thru presentations, health fairs, or other events per supervisor approval.  The incumbent will work closely with the Operations Services Manager to staff direct outreach activities throughout the community.  This may include, but is not limited to, distributing condoms, and visiting local businesses for the purpose of making literature and condoms available to the local community.  He/she will assist with clarification, resources development, and advertising of special events which will include STD Prevention Month, National Testing Day, National Latino Awareness Day, World AIDS Day, Black Awareness Day, and National Girl and Women Awareness day.  The incumbent networks with existing indigenous organizations to provide a comprehensive and culturally sensitive outreach programs within specific cultural/ethnic minority populations where HIV has taken a disproportionate toll on the community. Attends community meetings addressing HIV/AIDS issues as assigned.

 

Will act as Team Lead to the HIV/STD Outreach Team. Duties will include: maintaining daily activities in the department and ensuring staff coverage at mobile outreach events and the jail, conducting daily huddles to inform staff of daily assignments and share any DOH updates with the team, documenting total number of tests conducted at each outreach event and reporting totals to the division director daily, reviewing the annual workplan and ensuring activities are being met, ensuring the mobile unit is in good working condition, attending community meetings, and keeping the division director apprised of any issues or concerns that inhibit the success of the HIV and STD Outreach Team. 

 

The incumbent will ensure that the patients are knowledgeable about such programs as Housing Opportunities for People with AIDS (HOPWA), the AIDS Insurance Demonstration Project (AICP), the AIDS Drug Assistance Program (ADAP), Ryan White Title II, PAC Waiver and HIV/AIDS Case Management.  In addition, the incumbent assists patients on a limited basis to access social programs such as disability, food stamps, emergency food baskets, clothes, locating affordable housing and any other needs related to the patient's holistic care.  If the clients have a HIV case manager these services should be coordinated with their manager.  The incumbent works with patients to develop individualized plans of care to encourage compliance with their treatment/medication regimen and laboratory test. Maintains records of the number of hours with each client, number of units, and quarterly statistics to include sex, race, ethnicity, and income of clients seen.  Those HIV positive clients will also be educated on ways to reduce the incidence of HIV/STD transmission by evaluation of high risk behaviors and encouragement of safer behaviors including the mandatory informing of potential sex partners. 

 

Maintains a caseload of HIV positive patients, pregnant women, and their exposed children up to 18 months of age, and high risk HIV negative patients as assigned by supervisor.  This caseload is back-up to the Linkage to Care Coordinator. This position serves as back-up Linkage to Care Coordinator. Encourages healthy and safer behaviors to positive and high risk negatives clients, and ways to reduce HIV/STD transmission or reinfection at 100% of contacts either in person or by phone. Maintains accurate and clearly documents patient's record with contacts in person or by phone no later than 36 hours after event.  Maintain records of the amount of contact hours, number of clients and sex, race, ethnicity and income of clients seen, for monthly and quarterly reports, and submits to supervisor no later than the 3rd day of the following month.

 

This position serves as the Preventive and Training Counselor. The incumbent staffs, and helps to develop and plan activities for HIV/AIDS education & testing throughout Manatee County. He will be responsible for activities and classes.  Develops networking contacts in the community to expand the education programs to target populations for HIV/AIDS.  Develops and promotes educational and training opportunities and provides HIV 101, 104, and 500 classes.  Provides 501 trainer courses and updates. Acts as primary Health Educator for the HIV/AIDS program, and maintains own certification on a yearly basis. Maintains an up to date knowledge of HIV/AIDS and community resources through regular staff meetings, continuing education, reading, workshops, seminars, conferences, etc. 

 

Facilitates and recruit the Voices intervention making sure all the core elements are included and guidelines are followed.  Consults with and solicits community agencies, the private sector and community service organizations to assist with program development, expansion and improvement through their involvement.

 

Maintains an up to date knowledge of HIV/AIDS and community resources through regular staff meetings, continuing education, reading, workshops, seminars, conferences, etc. Ensures at least 4 hours of training a year, and competency in operating computers (including Word, Excel, Power Point, Publisher, etc.)

 

Reports accurate daily activities into the HMS on a weekly bases, and time for payroll to the People First System no later than 5:00 PM Thursday of closing period. Reports individual monthly report, to supervisor no later than the 3rd of the following month.  Maintains an accurate and timely calendar with daily activities.    

 

Performs other related duties as needed.

Qualifications:

Must have a valid Florida Driver’s License and reliable transportation

Certified Phlebotomy Technician (CPT) or other certification to draw blood
