This position is a clerical support position that reports directly to the Health Information Specialist Supervisor. This position is responsible for client processing and assisting nursing staff for assigned clinic. This position will be assigned to any clinic and to medical records, as needed, at the direction of the supervisor, to provide support to team members or coverage in their absence.




This position requires that you have at least two years of experience working in a professional medical or office setting.


Preference will be given to individuals who have knowledge of registration, Insurance and service data entry and clinical scheduling experience. Prefer experience determining eligibility for government assistance program, Record Releases, scanning documents into Electronic Health Record, customer service in public medical setting and with multi-line phone system.




Triage incoming calls and/or clients as necessary. Registers new clients in Florida Shots and HMS updated existing clients and schedules appointments for clinics. Ensures desk procedure for position is maintained. Prepares clients’ medical records for the clinic visit to include; verify clients’ demographic information and payor source for each visit, securing appropriate forms under the financial section of the medical record. Accurately date super bill and encounter forms, ensure clients’ financial eligibility status and program code are annotated on the super bill, refer clients to the financial counselor as appropriate. Secure appropriate forms and test results in the medical record specific for each clinic visit. Ensure forms are organized and labeled for clinic use.


Responsible for all operations concerning medical records; medical records workloads, work flows, deadlines, and work objectives with supervisor. Position handles incoming subpoena and medical records release. Prints laboratory manifest, incoming lab reports and logs reports received, ensures labs are routed to clinical staff following desk procedures. Ensures desk procedures for medical records are maintained. This position is a clerical support position that is responsible for client processing and assisting the assigned clinical team. Including but not limited to: Greets and registers new and existing clients. Updates client demographic data each visit using HMS system. Verifies payment source each visit to include; Medicaid with FMMIS system or appropriate economic evaluation or Insurance. Counsels client on HIPAA, information release forms for billing purposes and release of medical information, consent for care and contact information. Triage incoming calls and/or clients as necessary. Requests photo ID and retains a copy in the Medical record. Translates for clinic as needed, if applicable.


Performs duties as outlined in the FCHD Policy and Procedure Titled "Disaster Operating Procedure", and other related duties as assigned. Access to Social Security Data and Clinical information for services provided.
