Your Specific Responsibilities:
This is a professional healthcare role (RN) requiring the incumbent to serve across multiple Women’s Health clinics, delivering direct clinical support and leading health education initiatives for staff, clients, and community partners. Responsibilities include outreach and training efforts that promote public health awareness and access to care.
Clinical support areas encompass Women’s Health, Prenatal Services, Sexually Transmitted Infections (STIs), Immunizations, Refugee Health, and Tuberculosis (TB). In this dual-capacity position, the incumbent will collaborate closely with an experienced Advanced Practice Registered Nurse (APRN) to provide essential care and health education to women who may otherwise face barriers to accessing medical services.
A central focus of this role is the facilitation of the Diabetes Prevention Program (DPP)—an evidence-based initiative aimed at reducing the risk of type 2 diabetes through community engagement and education. The incumbent will be responsible for delivering program content, maintaining accurate reporting, promoting the program through targeted outreach, and serving as a liaison to local healthcare partners.
Additionally, the incumbent will act as a designated trainer for Department of Health (DOH) staff, specifically in the context of clinic operations during shelter activation or emergency response for special needs populations.
Specific Responsibilities:
Nursing services utilize direction and concepts adhering to the established Florida Nursing Standards per theFlagler County Health Department protocols, FL- DOH internal operating procedures and Technical Assistance guidelines, CDC, and Title X guidelines, working to meet the most current DOH goals. Application of nursing principles and techniques in the care and treatment of clients: client assessment, medication issuance, history collection, counseling clients on the provider's recommendations, arranging appropriate referrals and testing, appropriate documentation in the electronic health record and entering service data entry.
Duties encompass maximizing access to services, Prenatal/Family Planning, Case management and cross-training to various roles such as Immunizations, performing Pregnancy Testing and Counseling, Contraceptive initiation or appropriate referral, HIV testing and counseling, Sexually Transmitted Disease screenings and treatments, Tuberculosis screening and testing, insulin program, LTBI, Direct Observed Therapy and general clinic medication issuance. Performs and documents vital signs, venipuncture, fingerstick hemoglobin and glucose testing, urine testing using the chemistry strip, urine pregnancy testing, vision, and hearing screens.
Assists provider in the exam room for specimen collection and procedures. Set up, clean-up and restock examination rooms. Obtains, creates electronic orders, prepares, and packages laboratory specimens for shipping.
Keeps supervisor abreast of daily clinic activities. Perform daily employee activity record and time sheet reporting. Participates in workshops, in services, conferences, meetings and other activities related to public health. Participates in cross training and staff development activities for career development. Participates in quality improvement processes and programs. Performs other job-related duties as delegated by the nursing supervisor. Participates in record review for quality assurance. Maintains detailed records of cases and assemble these into periodic reports. Data entry and familiarity with computer software programs. This position requires an active Florida Registered Nursing License. Under general supervision, interviews,evaluates, monitors clients, provides education, guidance and isolation/quarantine parameters based on Centers for Disease Control and Prevention (CDC) as well as the Florida Department of Health in Flagler County recommendations for those individuals who meet case definition for a confirmed, suspect, or probable case of a communicable disease. Assist with coordination of training needs with senior nursing staff. Participate, plan, develop, and/or coordinate trainings as needed to enhance staff preparedness efforts, especially those relating to Special Needs shelter activities. Assists with the facilitation of Health Education classes and support groups. Works with the Healthy Communities to execute goals and objectives. Completes required paperwork, including reports, as designated by Health Education programs or grants. Assists with Health Education programs in promoting healthy lifestyles for clients and members of the community through special projects and duties as assigned, including outreach events in the community and at the Department of Health. Understands state-wide health initiatives and works toward implementation of local efforts to comply with county, state and federal goals and objectives. Assesses and immunizes clients as appropriate for specific programs adult, pediatric and travel programs. Records and establishes the description and timeline of onset of symptoms and interviews case to determine all possibly exposed individuals. Explains information and recommendations regarding isolation and quarantine parameters, services available to case, and monitors case for ongoing needs and disease clearance. You must be willing to work before, during, and/or beyond normal work hours or days in the event of an emergency This may include working in a Special Needs or Red Cross shelter, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. Other duties as assigned.
Required Knowledge, Skills, and Abilities:
Ability to establish and maintain effective working relationships with associates, health agencies, physicians, individuals, and the public.
Ability to evaluate and interpret epidemiological and statistical data pertaining to the control of communicable diseases.
Ability to present information clearly and concisely orally and in writing. Have an understanding of patient confidentiality, including the ability to conduct interviews without violating confidentiality (e.9., to those who might overhear their conversations).
Qualifications:
Minimum -
Must be willing to accept this salary if offered the position
Must have 6 months experience as an RN in a health care setting
Must have 6 months experience providing Health Care services in awomen's health setting
Preferred -
Resourcefulness in locating patients and contacts who may be difficult to reach or reluctant to engage in conversation
Understanding of when to refer individuals or situations to medical, social, or supervisory resources
Cultural competency appropriate to the local community
