Under the supervision of the Community Health Nursing Director this nurse is responsible for the Flagler County Health Department’s Immunization program, including vaccine administration, storage, handling, and accountability.  Responsibilities include childhood, adult, travel and post exposure prophylaxis immunizations, and the provision of TB and Refugee Health screenings through the Nursing Director.  Duties include home visits, direct observed therapy, outreach, other clinics as assigned, on- call rotation, disease outbreak and preparedness response.  Services are provided according to the Flagler CHD protocols, FL DOH policy and procedures and the CDC guidelines.

Implements Immunization program providing direction and concepts assuring adherence to established nursing standards and local Immunization policy and procedures.  Manages the Immunization clinic (adult, travel, and pediatric immunizations), TB screening and Refugee Health, Immigration access to services. Will be cross trained to Family Planning and Sexually Transmitted Disease clinics as a backup, to provide screenings and treatments) to include triages, direct services and participates with outreach activities. Maintains employee Immunization and Tuberculosis screening registry. Provides Immunization and Tuberculosis education and updates for employees. Provides Supervisor with monthly CASA report, maintains report according to state directives not to be below 95%. Provides on-call services according to schedule.  Provides Direct Observed Therapy for the Tuberculosis program.

Assesses, counsels and educates clients and members of the community regarding Adult, Childhood, and Travel immunizations to clients  according to Flagler CHD protocol, FL DOH and CDC guidelines.  This includes immunization data entry, generating immunization  certificates and records for the clients, families, and school. Documents activities in Electronic Health Record, Florida Shots Immunization Registry and completes service data entry into the HMS service module timely.  Runs CASA reports weekly, maintains a clean registry.  Runs aging vaccine report monthly to minimize vaccine wastage.

Screens and tests patients for Tuberculosis according DOH Guidelines and Nursing Protocols, utilizing the TB template in HMS. Ensures all clients are provided follow-up services as necessary according to the TB program office.  Provides Initial Refugees Nursing and TB assessments, coordinating care with clinics.

Provides direct support in the WHS ARNP clinic, FP and STI RN protocol and treatment clinics, when needed as back up.  Arranges clinic to make sure all patients are served.

Manages Vaccine Inventory according to CDC and FL DOH guidelines, to include refrigerator and freezer temperature reading,    uploading data results from the reading into the monthly Vaccine for Children vaccine inventory in FL Shots, ordering and tracking of vaccines. Sets up and takes down clinic, maintains quality assurance checks on clinic equipment.  Orders, receives, and inventories immunizations for all CHD programs. Participates in Outbreak Disease investigations as assigned. Assists School Health Nurse with outreach immunization programs. Provides support staff with direction as needed.

 Incumbent is required to complete time sheet, work activity reports (EARs) if applicable, and mandatory trainings. Trainings may be deemed mandatory by Agency, Statute, Grant, Program, or County Administrator. Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

 

Required Knowledge, Skills, and Abilities:

Electronic Health Record data entry, Vaccine Administration to infants, children and adults, on-line course - CDC TRAIN Immunization: You call the shots Florida Shots (or similar), CPR, Disaster Preparedness, Inventory and ordering Experience, Case management, TB case management training, HIV 500/501.

Qualification

Minimum –

Valid RN licensure in the State of Florida in accordance with  Florida Statutes.

One or more years’ experience as a Registered Nurse in health care setting.

One or more years’ experience working in a busy clinic setting and are you able to multi-task and work efficiently and accurately under pressure.

Experience as a Registered Nurse in performing a nursing assessment, providing education and/or providing any immunizations.

Nursing experience counseling teens, young adults, adults on immunizations.

                                                                                                               

Preferred –

Knowledge of Microsoft Office applications.

Ability to work in web-based programs.

Ability to work independently with minimal supervision.
