APD’s Developmental Disabilities Defendant Program (DDDP) Overview

 DDDP is a forensic facility working with individuals with disabilities in a secure setting that operates on the campus of Florida State Hospital in Chattahoochee, FL, where the campus and facilities are tobacco free.  

Pathways is a 34-bed all male forensic facility working with individuals with disabilities in a secure setting.  These individuals have been adjudicated by the courts as incompetent to proceed to trial and have significant behavioral and psychiatric issues.  Pathways provides rehabilitative, vocational, and daily living skills training to the individuals served which allows for a smooth transition back into a non-secure residential or community setting.

Position Summary 

This highly responsible professional, administrative position serves as the DDDP/Pathways Risk Manager/Safety Officer. The primary goal is to reduce, control, or eliminate the possibility of incidents to residents, staff, and visitors, thereby minimizing financial loss for DDDP/Pathways. This is achieved by assessing, planning, and coordinating responsibilities to prevent or minimize accidental injury, illness, property damage, or other preventable loss or negligent acts. This role is responsible for the overall coordination of the incident reporting system and the general liability program for DDDP/Pathways.

 

The Work You Will Do 

  This highly responsible professional, administrative position serves as the DDDP/Pathways Risk Manager/Safety Officer. The primary goal is to reduce, control, or eliminate the possibility of incidents to residents, staff, and visitors, thereby minimizing financial loss for DDDP/Pathways. This is achieved by assessing, planning, and coordinating responsibilities to prevent or minimize accidental injury, illness, property damage, or other preventable loss or negligent acts. This role is responsible for the overall coordination of the incident reporting system and the general liability program for DDDP/Pathways.

 

Risk Management & Incident Response:

•              Leads the identification, communication, measurement, and management of facility-wide risk.

•              Processes and analyzes a magnitude of daily resident incident reports in a priority manner, reviewing for timeliness, content, accuracy, and compliance with applicable Florida Statutes, Florida Administrative Codes, Department Regulations, and DDDP/Pathways Operating Procedures.

•              Develops, maintains, and monitors the DDDP/Pathways Risk Management Incident Tracking System, covering resident incidents, including the type, frequency, causes, and outcomes.

•              Analyzes the frequency, causes, general categories, and types of incidents causing injury to residents, making recommendations to reduce their frequency and severity.

•              Advises supervisors of any adverse patterns or trends identified in incident reports.

•              Completes applicable follow-up actions, makes necessary referrals, and maintains a suspense system to ensure timely completion of actions.

•              Manages all phases of the claims process from intake and investigation through litigation, settlement, and/or trial.

•              Investigates, resolves, and responds to all property and liability claims under the direction of the Chief Administrative Officer & General Counsel.

•              Provides timely notification of claims to insurance carriers and develops and communicates plans to address loss events.

•              Manages the workers' compensation and incident reporting program, processing, assessing, and reporting claims to carriers as appropriate.

•              Partners with all departments regarding incident reports and claims or losses.

•              Conducts post-event reviews, including conducting meetings, video reviews, and preparing summary reports.

Safety Officer & Compliance:

•              Serves as the DDDP/Pathways Safety Officer and Chairperson for the Safety Committee, holding monthly meetings, distributing minutes, and tracking resident safety action items until completion.

•              Promotes health and safety of patients and employees, reviews and evaluates the quality of patient safety measures, and assists in implementing the DDDP/Pathways safety plan.

•              Ensures the development of appropriate operating procedures that minimize the risk of injury to residents and employees.

•              Ensures compliance with risk management Florida Statutes, Florida Administrative Codes, Florida Department of Health, the Safe Medical Devices Act, and Agency for Persons with Disabilities Regulations.

•              Functions as a subject matter expert for Fire and Safety, OSHA, ADA, and Life Safety Codes.

•              Completes Facility Site Inspections, including quarterly safety site visits at DDDP/Pathways, to ensure occupancies and emergency safety procedures are followed as required by local and federal construction safety, life safety codes, building fire codes, labor and environmental laws, and sanitation procedures. Identify potential hazards, operational gaps, and vulnerabilities in physical locations (e.g., room/door integrity, perimeter security).

•              Completes QA facility walk-throughs, reports findings, and monitors corrective actions related to facility inspections.

Emergency Preparedness & Operations:

•              Implements and updates Emergency Operations Plans for DDDP/Pathways to protect and preserve the safety of residents and staff by developing, testing, and executing emergency preparedness plans for various scenarios (e.g., riots, natural disasters, medical emergencies, lockdowns).

•              Conduct drills and table-top exercises to train staff and identify areas for improvement.

•              Coordinates efforts with local emergency managers for emergency response efforts.

•              Collaborate with the staff development department to raise awareness of safety preparedness and response.

•              Acts as a Liaison for Departmental Operations involving emergency/crisis situations and works with the state office to provide tracking, census, and requested updates.

Policy & Program Development:

•              Develop and implement risk management and compliance policies/procedures.

•              Partners with key stakeholders to develop and implement standards, processes, programs, and best practices related to risk management.

•              Identifies potential risk exposures, recommends solutions, implements approved programs, promotes loss prevention, and updates and monitors compliance through audits and reviews.

Reporting & Training:

•              Produces and provides monthly/quarterly/annual comprehensive statistical reports to Administration including information about all risk management activities.

•              Produces monthly, quarterly, and annual analytical reports for residential units at DDDP/Pathways (e.g., incident reporting, workers comp, certification compliance, etc.).

•              Ensures Risk Management pre-service training is presented to all new employees within 30 days of employment pursuant to the Florida Administrative Code.

•              Provides Risk Management and Emergency Operations annual update training to all employees.

•              Audits and tracks all professional certifications.

Insurance & Third-Party Management:

•              Manages insurance procurement and evaluates and provides recommendations regarding the selection of policies, including but not limited to: Property, Casualty, General Liability, Workers’ Compensation, and Directors and Officers policies.

•              Coordinates and submits, as necessary, the gathering of insurance policy renewal data.

•              Manage relationships with third-party service providers, including brokers, underwriters, and other third-party administrators.

•              Works with the company’s insurance brokers to respond to requests for certificates of insurance and claims history documents.

•              Works in partnership with the General Services manager to ensure appropriate documentation for insurance coverage for all buildings and building contents.

Special Assignments:

•              Performs special assignments such as researching, gathering information, analyzing, and stratifying data.

•              Participates in ward government meetings.

•              Completes additional duties as assigned.

Minimum Qualifications 

  • A valid driver’s license.

•              Bachelor’s degree from an accredited college or university and two years of professional experience with quality improvement and/or other related project management skills.

•              DDDP/Pathways are located at two different campuses.  (Chattahoochee, FL and Marianna, FL).  The employee office will be located in Chattahoochee, FL but may be required to do work at both locations depending on the project or responsibility at hand.

 

Additional Requirements for This Position

  • Certification in the Agency’s Crisis Prevention and Intervention Program. Advance training may be required.
  • A pre-employment and annual TB screening.
  • Annual CPR Certification.
