Your Specific Responsibilities:

The Human Services Analyst will provide clerical assistance to the unsheltered homeless where they reside as part of a street medicine initiative of the Health Care for the Homeless Program. The incumbent will be responsible for engaging potential clients, helping with eligibility applications, organizing clients on a first come first serve basis, entering data in data bases and excelt spreadsheets, and scheduling appointments. The work is field-based and will go out into the community to engage with the homeless population. 

 

  • Completes daily engagement of unsheltered patients in the community and clinic setting, such as handing out Narcan and other harm-reduction items as directed by clinical team.
  • Assists patients with obtaining community supports and resources as needed and in conjunction with the Directions for Living Case Manager.
  • Uses multiple computer applications to perform duties and keeps technical skills currents which includes Next Gen, Carescope, maxRTE, Eaglesoft, and HMS.
  • Interviews and determines financial eligibility for services based on established criteria for new and established clients.  
  • Registers and checks in clients
  • Monitors daily Street Medicine schedule and communicates with clinic staff as needed
  • Photocopies and/or faxes medical record based upon appropriate requests; ensures medical record releases are      
  • Serves as the primary driver of the Street Medicine vehicle, ensuring timely arrival at scheduled locations daily. Responsible for daily set up and break down, fueling, and ensuring cleaning of the Street Medicine vehicle.

 

Required Knowledge, Skills and Abilities:

  • Ability to plan, organize and coordinate work assignments.

  • Ability to establish and maintain working relationships with others.

  • Ability to type and/or in a keyboard control data input device, adding machine and utilize other office equipment.

  • Ability to follow instructions.

  • Skill in working effectively with the public.

  • Knowledge of computer systems.

  • Ability to communicate effectively both verbally and in writing.

  • Have dependable transportation. 

Qualifications:

Minimum-

  • Access to reliable transportation
  • Experience working in a medical clinic environment, including knowledge of medical terminology
  • Three or more years of customer service experience
  • Three or more years of data entry experience

 

Preferred-

  • Three or more years of experience determining financial eligibility for services
  • Experience working with the homeless population
  • Experience providing services in an outreach setting
