OPS Human Services Analyst

 

This position will provide dual support with vaccine outreach and clerical assistance to the unsheltered homeless where they reside as part of a street medicine initiative of the Health Care for the Homeless Program. The incumbent will be responsible for engaging potential clients, researching vaccine history in Florida Shots, helping with eligibility applications, organizing clients on a first come first serve basis, entering data in data bases and excelt spreadsheets, and scheduling appointments. The work is office based as well as going out into the community to engage with the homeless population. 

 

Your Specific Responsibilities:

Drive to vaccine engagement and street medicine locations
Assist clients with forms and applications 
Conduct eligibility determination and enrollment into services based on established criteria
Confirm and updates client demographics
Coordinate patient appointments
Provide information on and refer individuals to public or private agencies and community services for assistance
Keep updated on community resources for individuals experiencing homelessness
Keep records and prepare reports as required

Coordinate ordering of supplies
Maintain client confidentiality in accordance with Florida statutes
Demonstrate good interpersonal communication skills via in-person and/or telephone encounters
Use multiple computer applications to perform duties and keeps technical skills current which may include: NextGen, CareScope, HMS, MaxRTE and Florida shots

Send medical records requests as needed to participating partners, specialists, and hospitals as requested by providers
Adhere to established safety procedures and practices
Ensure medical record releases are completed and filed appropriately
Answer all telephone calls courteously and promptly with accurate information          
Maintain resources for client information 
Attend team and other meetings as assigned  
Keep supervisor and team members informed                  
Perform other related duties as assigned including emergency duty

 

Required Knowledge, Skills, and Abilities:

Qualifications:

Minimum-

  • Access to reliable transportation

 

  • Experience working in a medical clinic environment, including knowledge of medical terminology
  • Three or more years of customer service experience

 

Preferred-

  • Three or more years of data entry experience
  • Three or more years of experience determining financial eligibility for services
  • Experience working with the homeless population
  • Experience providing services in an outreach setting
