OPS Human Services Program Specialist

 

This position is for the HIV PrEP Coordinator and patient navigator for the Health Care for the Homeless Program serving patients at the Bayside Health Clinic and Mobile Medical Unit. This position involves regular contact with a diverse population both in person and over the phone and requires working with sensitive and confidential information.

 

Duties and Responsibilities:

  • Receives client referrals from clinic staff, HIV/ Disease Control Program staff for partner referrals of HIV positive clients, as well as direct inquiries by phone or walk-in from clients.
  • Conducts client-centered PrEP education.
  • Assists PrEP clients with enrollment into medication assistance programs and scheduling appointments with health care providers.
  • Coordinates with clinic providers for laboratory tests and prescription orders. Collects data in preparation for PrEP services.
  • Carries a caseload and conducts follow-up calls and appointments to address client adherence to appointments, lab tests, and medication.
  • Participates in outreach activities to promote HIV testing and PrEP education and services with the program and in the community.
  • Participates in educating staff members on use of PrEP.
  • Attends meetings and trainings as assigned.
  • Prepares reports and presentations as assigned
  • Performs all other duties as assigned.

 

Knowledge, skills and abilities, including utilization of equipment, required for the position:

  • Knowledge of varied social, economic, cultural and lifestyle characteristics, and a skill in communicating with, and eliciting information from patients and target groups with these diverse backgrounds.
  • Knowledge of public health systems, as well as, professional and interpersonal skills to interact with health care clients, health care professionals and others who impact STI prevention and control activities, including community partners.
  • Knowledge of Information Security and Confidentiality statutes, rules, regulations, policies and procedures.
  • Employee must successfully complete HIV/AIDS 501 training within 6 months of employment.
  • Effective written and verbal English communication skills.
  • Effective interpersonal skills.
  • Ability to perform basic computer operations.
  • Knowledge of computer software applications.

 

Minimum Qualifications:

  • Must have valid driver's license or the ability to obtain one within 30 days of employment.
  • Must have reliable personal motorized transportation, i.e. automobile or similar vehicle, to perform outreach activities and travel between CHD locations as needed.
  • Must have recent experience conducting client-centered PrEP, nPEP, and/or STI/HIV prevention education.
  • Must have recent case management and/or patient care coordination experience.

 

Desired Qualifications:

  • Experience working in a medical clinic environment, including knowledge of medical terminology and use of an EHR.
  • Experience working with the homeless population.
  • Experience with public speaking and commnity outreach.
  • Expereince providing rapid HIV testing.
  • Expereince providing training and education in a health care setting.
