Your Specific Responsibilities:

This is a highly responsible and independent position working for the Department of Health Lake County under the direct supervision of the Florida Department of Health Lake County Assistant Community Health Nursing Director (ACHND). This position also works closely with and coordinates with the Epidemiology Team regarding matters related to Epidemiology and the Assistant Nursing Director on matters regarding Tuberculosis prevention and control.  This position supports the Perinatal Hepatitis B Program Coordinator conducting case investigation and providing case management services for infected pregnant mothers and exposed infants.  This position will provide support to the Epidemiology program coordinating clinic services for Rabies Post Exposure Prophylaxis clients and conducting case investigation and date entry services as assigned.

 

Incumbent will support the Tuberculosis Program Health Educator (ACHND) coordinating the caseload of Active, LTBI, EDN and +QT clients and their contacts.  Responsibilities will include case investigation and case management services including health education on transmission and contact investigation, Incumbent will provide Direct Observe Therapy and Video Direct Observe Therapy as ordered (weekly, daily) and document medication adherence in HMS tracking modules.  Incumbent will coordinate Labs, X-rays, and monthly assessment appointments with the clinic staff.  Incumbent will work with Assistant Director of Nursing with care plan documentation and client compliance as needed.

 

Incumbent will support the Perinatal Hepatitis B Program Coordinator (ACHND) coordinating the caseload of HBsAg + positive pregnant
women and their affected contacts. Responsibilities will include case investigation services to detect newly pregnant HBsAg positive women and provide case management services including health education on transmission and prevention, contact investigation and vaccination services, request and management of confidential records, and infant follow-up. Incumbent will gather, compile, and enter
data into the MERLIN system and will maintain data quality controls in coordination with Epidemiology Snapshot objectives.

 

Will provide support to the Epidemiology program coordinating clinic services for Rabies Post Exposure Prophylaxis clients and
conducting case investigation and data entry services as assigned.

 

Incumbent will conduct community outreach and engagement activities as assigned.  The incumbent in this position may be required to use vehicles owned, leased, or operated by the state or county to conduct official state business. State or county vehicles shall be used only for officially authorized duties and not for personal use, unless a specific exception applies under Florida law (F.S. § 287.17).

 

Performs other related duties as assigned.

 

Required Knowledge, Skills, and Abilities:

Customer Service, Computer Programs-Data Entry, Use of Excel, Word, Outlook, use of Fax, and Copier.

 

Qualifications:

Minimum –

 

  • Willing to work before, during, and/or beyond normal work hours or days in the event of an emergency. This may include working in a special needs or Red Cross shelter, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
  • Current Florida Registered Nursing License or a Registered Nursing License from another compact state.  An official current Registered Nursing License is needed to confirm your qualifications for this position.
  • Willing to travel to different Department of Health sites in Lake County as needed.
