Your Specific Responsibilities:
Duties and Responsibilities
This is an advanced practice nursing position responsible for medical management, under protocol, for clients seeking women's health, including prenatal care, family planning, and STD services at Walton CHD. This position works with the Medical Director and designees in the planning, coordinating, and implementing of public health clinical services and in the development of related policies and protocols. This is a sensitive position requiring background check, including fingerprinting and urine drug screening. This position is key custodian for the medication room and other assigned areas. This position works under the general supervision of the Medical Executive Director. This position adheres to Information Security policies and procedures.
This position will provide patient-centered interactions:
• Respect patient and family values and expressed needs.
• Encourage patients to expand their role in decision-making, health-related behaviors, and self-management.
• Communicate with their patients in a culturally appropriate manner, in a language and at a level that the patient understands.
• Provide self-management support at every visit through goal setting and action planning.
• Obtain feedback from patients/family about their healthcare experience and use this information for quality improvement.
This position will assist in the provision of care coordination:
• Link patients with community resources to facilitate referrals and respond to social service needs. Integrate behavioral health and specialty care into care delivery through co-location or referral protocols.
• Track and support patients when they obtain services outside the practice.
• Obtain test and referral results and communicate with community organizations, health plans, facilities, and specialists.
• Follow-up with patients within a few days of an emergency room visit or hospital discharge.
• Communicate test results and care plans to patients/families.
This position will follow Walton Community Health Center Patient-Centered Medical Home policies and procedures for delivering exceptional care to patients.
- Following protocols, policies, and procedures, performs complete examinations, appropriate to patient need, of patients seeking women's health, including family planning, prenatal services, colposcopy, and STD services.
- Acts as senior provider for prenatal care.
- Compiles and evaluates medical histories and physical findings to identify, evaluate, educate, and make treatment recommendations related to health needs, including preventive services.
- Selects and initiates diagnostic laboratory tests and interprets and evaluates results in a timely manner.
- Monitors medication and initiates and alters therapies according to patient response.
- Maintains accuracy in the medication list and problem list of the patient's chart.
- Works independently at clinic sites without direct supervision. If assigned, works independently during extended hours and weekend hours without on-site supervisor.
- Provides consultation, technical assistance and leadership in evaluating the appropriateness of client care, in design and development of health educational materials, and in personnel training of nursing and other ancillary staff.
- Coordinates patient care with other health professionals.
- Helps in the development of clinical protocols for prenatal and family planning.
- Member of assigned QA/QI workgroups and projects to support the organization’s Strategic Plan and initiatives.
- Monitors quality assurance/improvement procedures for family planning, prenatal and colposcopy services.
- Precepts nursing students/ARNP students as requested.
- Attends appropriate staff meetings.
- Attends conferences and trainings to enhance job skills and to meet licensure requirements.
- Completes all required reports, including EARS within required time frame.
- Performs other related duties as required.
- Duties during times of disaster are required as indicated to meet the needs of the citizens of Walton County.
May be assigned purchasing card responsibilities; if so, will adhere to purchasing card policies and procedures.
Required Knowledge, Skills, and Abilities:
Medical training as a Woman’s Health APRN or other APRN training with equivalent woman’s health experience. Colposcopy training and experience. Knowledge of prenatal care. Supervisory ability and experience. Familiarity with family planning program requirements. Ability to stand for 8 to 10 hours with brief breaks. Ability to use a computer for data retrieval and entry. Ability to communicate with patients and staff in both verbal and written fashion. Ability to lift greater than 25 lbs occasionally. Ability to push and pull and to work overhead as needed for examination of patients. Ability to wear mask or another respiratory protective device. Ability to wear sterile gloves and other exam gloves on a frequent basis. Ability to function in high stress, noise, and distractions of clinic and emergency environments. Ability to adjust to changing demands and duties. Excellence in interpersonal skills. Ability to de-escalate tense interpersonal communications with others.
Qualifications:
Minimum - Licensure as registered nurse pursuant to FS 464,Valid Florida licensure as an APRN
