Your Specific Responsibilities:

This is a sensitive position conducting executive level work planning, directing, or coordinating activities in the medical clinic. Incumbent to provide medical and general oversite of clinical services and other programs/areas which will require medical expertise/decisions in a public health setting and for the provision of medical services to the citizens of Walton County. The incumbent in the position will be a direct report of the Walton County Health Department Administrator. He/She is responsible for the integrity and confidentiality of data sets under his/her control. Responsible for the supervision of staff including appraisals, planning and directing their work, approving leave, and having the authority for hiring and taking disciplinary action or effectively recommending such actions. Key custodian for assigned areas. This position will assist in the management of the WCHD clinical services, which will include coordinating or assisting in the development of clinical program policy. Adheres to Information Security policies and procedures.

 

This position ensures that a patient-centered medical home approach to providing total health care to patients is used. Patient-centered is a way of saying that the patient is the most important person on the health care team.  The patient is at the center of the health care team, which includes health care professionals, trusted friends or family members, and the patient.

  • Provide patient-centered interactions:
  • Respect patient and family values and expressed needs.
  • Encourage patients to expand their role in decision-making, health-related behaviors, and self-management.
  •         Communicate with their patients in a culturally appropriate manner, in a language and at a level that the patient understands.
  • Provide self-management support at every visit through goal setting and action planning.
  • Obtain feedback from patients/family about their healthcare experience and use this information for quality improvement.
  • Provide care coordination:

•      Link patients with community resources to facilitate referrals and respond to social service needs.

•      Integrate behavioral health and specialty care into care delivery through co-location or referral protocols.

•      Track and support patients when they obtain services outside the practice.

•    Obtaining test and referral results and communicating with community organizations, health plans, facilities, and      specialists.

•     Follow-up with patients within a few days of an emergency room visit or hospital discharge.

•    Communicate test results and care plans to patients/families.

  • Follow Walton Community Health Center Patient-Centered Medical Home policies and procedures for delivering exceptional care to patients.

 

Provide medical consultation and oversight to Advanced Registered Nurse Practitioners who render medical services under mutually developed protocols. Provide oversite and make recommendations for the provision of services relating to Federally Qualified Health Center.

 

Provide medical consultation and oversight to credentialed/protocoled nurses who render designated medical services under mutually developed protocols.  Example:  Medical Standard of Care Protocols.

 

Monitoring and evaluation of medical services to ensure they reflect professionalism and agency standards. Responsible for QI/QA process and evaluation of clinical services for the Health Department and Community Health Center.

 

Perform medical review and evaluation of tuberculosis cases through medical records reviews and patient related consultation with TB nurse case manager related to patients suspected of having TB or with diagnosis of active TB.  Participation in overall health department efforts in disease surveillance (prevention and control measures) and consultation for communicable and non-communicable diseases.

Assisting in the interpretation and collection of epidemiologic data in the health department.

Medical oversight in disaster preparedness both within the health department and also with other agencies involved in such efforts.

 

Provide direct medical care to patients of the Walton county Health Department to include: preventive medicine through screenings, examinations, diagnosis, and treatment related to family planning, and STD patients, and the provision of primary care services to those patients seeking at Walton CHD.

 

Member of Leadership Team. Serve on various teams to support the Health Department and Community Health Center.

Emergency duty is required. Department of Health has responsibilities during and after emergencies. This DOH position may be required to work during or beyond normal work hours and/or days performing emergency duties. These duties may require working in abnormal conditions and/or potentially hazardous environments.

Qualifications:

Minimum - Licensure and a physician pursuant to FS Chapter 458 or 459, Valid Driver’s License, &

Current CPR Certification
