Your Specific Responsibilities:
This position is under the general supervision of the Community Health Nursing Supervisor. This position is identified as a sensitive position and may be responsible for receiving personal bills or performing tasks involving high level of trust. This position requires a background screening including fingerprinting. This position is identified as a key custodian for designated areas. Position will adhere to the Information Security Policies and Procedures.
Interview clients for FDOH Walton program eligibility following information security policies and procedures for all health department programs. Obtain vital information for patients/client integration in HMC system. Input demographics, financial data, and record information in HMS system for reports and patient care. Verify incomes and information for completeness and accuracy. Verify Medicaid eligibility using FMMIS. Prepare patient records according to policies and procedures. Determine eligibility for Presumptive Eligibility for Pregnant Women for women with positive pregnancy test following the Medicaid/PEPW guidelines. Complete applications using the Florida System to generate immediate temporary Medicaid.
Provide clerical support to Family Planning and Maternal Health programs of the Walton CHD. Maintain smooth clinic flow by adhering to the daily appointment schedule. Retrieve and prepare medical charts for clinic sessions adhering to records management guidelines. Verify/update patient/client information for HMS database. Provide routine information to Prenatal and Family Planning patients/clients regarding appointments, programs, clinic schedules, procedure, etc.
PROVIDE Healthy Start Initial contacts, care coordination, education and case management including home visits to all prenatal women, and families, who reside in Walton County, who are determined through the Initial Assessment to need Healthy Start services. Will provide services in accordance with client’s level of need following Healthy Start Standards and Guidelines. Maintains timely client records, assists in collection of statistical data and evaluation of information to assure objectives are met and standards are maintained. Provides parent education in prenatal care, parenting skills and infant child health. Assist non-English speakers with scheduling appointments with primary care as needed
Complete initial assessment (IA) screen for assigned Healthy Start (HS) clients. Ensures that prenatal clients are managed in accordance with HS pathway schedules. Provide psychosocial assessments, ASQ’s, Domestic Violence Screening. Provide appropriate referral to available resources as indicated. Develop a family support plan to meet identified needs. Document all services accurately into the Well Family System adhering to Healthy Start Standards and Guidelines.
Provide interpreter service for Hispanic clients of Walton County Health Department, assist in making appointments for those clients with other agencies in the community or health care providers to facilitate their health care
Complete all reports, including EAR within require time frame. Duty during disater required including work for irregular and indeterminate periods of time. Other assigned duties as needed to support clinic operations. Will be assigned purchasing card responsiblities related to travel. Assigned purchasing card responsibilities and adheres to purchasing card policies and procedures.

Qualifications:
Minimum - Valid Driver’s License, Fluent in Spanish, clerical experience
Preferred – Previous Healthy Start experience, experience with maintaining medical charts, experience with Medicaid/PEPW applications and eligibility
