Your Specific Responsibilities:

This is a clinical services position at the Florida Department of Health in Walton County providing practical nursing support and assistance to providers primarily in the Family Planning and Prenatal programs. All work will be completed  in accordance with agency policies, procedures, protocols, and guidelines. This position requires access to sensitive information and requires a high level of trust. This position is a key custodian for the medication room, laboratory, medical records room and other assigned areas. Adheres to Information security policies and procedures.  This position is based at the DeFuniak Springs site but may be assigned to other clinic locations or programs as needed for staff coverage.

This position will provide patient-centered interactions:

    • Respect patient and family values and expressed needs.
    • Encourage patients to expand their role in decision-making, health-related behaviors, and self-management.
    • Communicate with their patients in a culturally appropriate manner, in a language and at a level that the patient understands.
    • Provide self-management support at every visit through goal setting and action planning.
    • Obtain feedback from patients/family about their healthcare experience and use this information for quality improvement.

 

This position will assist in the provision of care coordination:

    • Links patients with community resources to facilitate referrals and respond to social service needs.
    • Tracks and supports patients when they obtain services outside the practice.
    • Obtains test and referral results and communicates with community organizations, health plans, facilities, and specialists.
    • Follows-up with patients within a few days of an emergency room visit or hospital discharge.
    • Communicate test results and care plans to patients/families.
    • This position will follow Florida Department of Health, Florida Department of Health Walton and Walton Community Health Center policy and procedures.
  • This position may provide immunizations to both children and/or adults:
  • Requires knowledge of immunization schedules, administration techniques, and use of VIS statements, internal procedures and computer system for data entry. May participate on both internal and external immunization events.

 

 

                                                                        Practical Nursing Tasks

Direct Client Care

  • Performs Hearing and Vision Screens
  • Performs EKGs and other in-house diagnostics.
  • Provides phlebotomy and specimen collection services.
  • Assists Provider with bedside procedures, for example, IUDs and Colposcopies.
  • Coordination of referrals for male / female sterilizations.
  • Assists with scheduling prenatal appointments and referrals as needed.
  • Administers oral, intramuscular, topical, subcutaneous, and inhalation medications / Vaccines.
  • Provides educational counseling and educational brochures to clients as indicated by health care needs. 

Triage and Work Up

  • Obtains and documents Vital Signs, Height, Weight, Body Mass Index (BMI)
  • Obtains Chief Complaint/History of Present Illness, Medical History, Allergy History, Medication History, and reconciliation of Current Medication List
  • Prepares client/exam room for provider visit.
  • Reports abnormal findings to provider.
  • Responsible for efficient clinic flow by working at a speed that maximizes provider productivity and minimizes client wait times.

Documentation

  • Records Objective and Subjective Data in Electronic Health Record as required in HMS, Florida Shots, Emdeon, and E-Lab for each designated program.
  • Records Objective and Subjective Data before the end of the clinic visit.
  • Performs Chart Review as directed for QA/QI.
  • Documents lab work in appropriate lab binders and in HMS.
  • Captures client services on the superbill and/or HMS at each visit.
  • Documents DAP medication pick up in HMS as needed. 
  • Completes HEDIS/UDS measure tool for clients prior to each visit.
  • Addresses and/or completes provider driven follow up list daily.

Facility Maintenance

  • Assures necessary supplies are always readily available in assigned clinic exam rooms and in the clinic procedure room.
  • Cleans exam rooms and direct patient care equipment after each visit and ensures clinic exam rooms are client ready before each visit.
  • Responsible for stocking work areas/clinic rooms. Responsible for ensuring that all supplies are in good date and condition.
  • Empties trash and disposes of supply boxes and or containers.
  • Secures full sharps containers, stores full sharp containers in designated area, and replaces containers as needed in clinic areas.
  • Cleans spills immediately when observed or discovered.
  • Immediately reports non-functioning or broken equipment or needed supplies to direct supervisor.

Laboratory Duties (may function as a back-up lab nurse)

  • Collects laboratory specimens per provider’s orders (blood, urine, sputum, stool, culture swabs).
  • Labels specimens.
  • Enters client data for lab order into HMS for processing.
  • Stores specimens according to laboratory requirements prior to shipping.
  • Use of lab equipment including centrifuge, autoclave, glucose machine, hemoglobin machine.
  • Performs in house labs as ordered and documents in HMS and in services.
  • Follows up on lab specimen results that are not timely.
  • Maintains testing logs and manifests.
  • Adheres to the use of personal protective equipment in the clinical and laboratory setting as required by policy and procedure

Other

  • May perform other duties within their nursing scope of practices as directed.
  • Attends and participates in monthly general staff meetings and departmental meetings.
  • Completes all job-related assigned trainings.
  • Completes Employee Activity Reports and submits timesheets according to agency standards.
  • Performs other duties as assigned.

  • Ability to use and maintain clinic equipment such as the centrifuge, autoclave, EKG machine, nebulizer, pulse oximeter, manual and digital scales, stadiometer, audiometer, vision screening machine, glucose machine, eye wash station, electronic blood pressure machine, manual blood pressure cuff, andHgbA1C and hemoglobin machine.
  • Ability to Stand, Sit , walk for extended periods of time.
  • Ability to lift and carry up to 25 pounds

Working hours:  Daily from 7:30 AM to 4:30 PM Monday thru Friday.  Total hours in workweek 40.  Explain any variation in work (split shift, rotation, etc.):  May be required to work extended clinic hours.

Qualifications:

Minimum - Licensure as a Licensed Practical Nurse per Florida Statutes 464, Current state issued driver’s license, Current CPR certification.

Preferred – Seasoned LPN with prior family planning. Woman’s health and/or prenatal care experience. Prior FDOH experience is a plus but not required.
