Specific Responsibilities:
This is a Nursing Program Specialist position in the Delray Beach Health Center supporting the department and /or organization and works with others in an effort to accomplish the goals of the unit, department and/or organization. Work is performed under the supervision of the Community Health Nursing Consultant. This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions. Adheres to established Health Department policies and procedures. Promotes the department as a positive, caring community organization through courteous behavior toward all clients and co-workers.
This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols, and Procedures. Confidential Information Sets: Clinical Information Data. This position may view, update or release “Confidential” information. This position has access to Q-Flow, Florida Shots, Health Management Systems (HMS), TRAIN, Surveillance Tools & Reporting System (STARS), TB, HIV, Emdeon, FDOH Public Health Pharmacy ordering / inventory system, and any other electronic health system as it relates to the position.
Responsible for participating in the State Pharmacy program at the local level using the FDOH State Pharmacy Forms System (PFS) to order, receive issue, distribute stock, reorder patient specific medications, and maintain a perpetual inventory for all medications used by the county health department. Pharmaceuticals include Nurse issuance bulk orders and patient specific prescriptions using the HOMER pharmaceutical system.
Work in the TB and HIV clinics and share patient workload.
Provide assessment, education, intervention and re-assessment of all assigned patients.
Provide medications as ordered.
Enter medications into Electronic Health Record (E.H.R.).
Complete lab requisitions for future internal and external appointments.
Coordinate patient’s follow ups and referrals.
Coordinate, case manage and maintains the patient specific registry for HIV medication regime for participating IDC clients.
Work in collaboration with all other clinic staff to ensure an efficient flow.
Practice standard precautions at all times.
Assist with clinic medication room coverage assignments. Act as the backup for the Pharmacy.
Case Management and coordination of patient treatment for IDC clients.
Work with colleagues to ensure patient eligibility for HIV services.
Participate in new programs for HIV.
Coordinate prior authorizations from patients’ insurance providers to ensure patients receive services & Medication as ordered.
Ensures linkage for pregnant HIV patients with County Perinatal Nurse.
Assist with coordination of new Test and Treat program in collaboration with the HIV program staff.
Links patients to ancillary services such as ADAP, nutrition, mental health and adherence counselors.
Coordinate treatment and case manage eligible Hepatitis C patients.
Attend regular HIV coordination meetings.
Conducts home visits when necessary for clients who are non-complaint or have critical labs/ medically comprised.
Manage TB care coordination and transfer of interjurisdictional care to appropriate Counties and State.
Management of Electronic Disease Notification (EDN) referrals and refugee referrals.
Electronic transmission of X-ray referrals to State consultant.
Continuous liaison with local TB program staff.
Takes precaution to reduce potential infection exposure when dealing with infectious cases coming to clinic.
Assists in contact investigation with TB program staff as needed.
Coordinate Direct Observe Prevention Therapy (DOPT) services as ordered.
Participates in new Antidote Project.
Completes all necessary training at State and CDC levels to ensure competency.
Attends TB consortium meetings on a rotational basis.
Prepared for and participated in local and State audits including TB, HIV, and PFS programs.
Ensure customer satisfaction by fostering a pleasant work environment and immediately reporting any issues to supervisor.
Assist in data collection.
Communicate regularly with supervisor to keep her/him aware of any issues.
Reports any malfunctioning equipment to supervisor.
Conduct quarterly phlebotomy classes for nurses and support staff to maintain competency and ensure consistent, high quality specimen collection practices.
Participates in Quality Assurance activities and in planned educational sessions.
Participates in staff meeting
Assist with new staff orientation and facilitate student preceptorship
Perform other duties as assigned.
Required Knowledge, Skills, and Abilities:

Qualifications:
Licensure/registration/certification requirements:
- This occupation requires licensure as a Registered Professional Nurse in accordance with Chapter 464, Florida Statutes, and in accordance with Rule 64B9 Florida Administrative Code.

- Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
- Valid Driver’s License.
- Must have a maintain a Basic Life Support Certification.
