Position Title: OPS Health Educator, Position Number: 64910118

Your Specific Responsibilities:

Seeking a skilled and motivated Health Educator to join the Community Health Division team. The ideal candidate should have a strong background in public health, data analysis, program evaluation, and familiarity with chronic disease management. As a Health Educator, the candidate will play a vital role in planning, implementing, and evaluating health education programs to promote community wellness, prevent diseases, and improve community health outcomes.

Responsibilities include:

  • Utilize problem-solving skills to identify, develop, and disseminate culturally and literacy-appropriate health education resources to target audiences.
  • Plan, implement, and evaluate community health programs based on community needs assessments, using evidence-based practices to achieve program objectives.
  • Provide tailored education to individuals, families, community groups, and stakeholders, considering their unique needs, lifestyle, culture, and socio-economic status.
  • Deliver targeted, culturally appropriate lectures and training to enhance understanding of health promotion, chronic disease prevention, policies, regulations, and local codes.
  • Conduct workshops, presentations, and training sessions for individuals, families, and community groups.
  • Provide health education to community partners through lectures and training sessions focused on targeted initiatives.
  • Advocate for vulnerable individuals and populations by assessing and evaluating healthcare services, ensuring awareness and accessibility of available programs.
  • Guide community members in accessing health resources, services, and support programs.
  • Assist in implementing and tracking strategies, goals, and objectives outlined in the community health improvement and strategic plan.
  • Serve as a resource for community partners, offering education on Community Health Improvement Plan (CHIP) initiatives.
  • Stay updated on current public health trends, policies, and best practices to ensure effective program delivery.
  • Evaluate the effectiveness of health education programs and recommend improvements

           

Required Knowledge, Skills, and Abilities:

The essential job functions of this position require excellent communication, presentation, and interpersonal skills, strong organizational skills, and attention to detail, the ability to work independently and manage multiple tasks simultaneously, strong analytical and problem-solving skills, intermediate knowledge or greater of Microsoft Office applications, familiarity with health disparities, social determinants of health, and public health principles. Must have a least one year of verifiable experience working in public health. Must be willing to work evenings and/or weekends as needed, in order to participate in community meetings, and to accommodate outreach activities. Ideal candidates will have an understanding of the Community Health Improvement framework and processes and the Community Health Assessment and Prioritization processes, familiarity with Health Insurance Portability and Accountability Act (HIPAA), and possession of a valid state unrestricted driver’s license.                

Qualifications:

Minimum –

  • Must have at least an intermediate level of knowledge of Microsoft Office applications including Excel, and PowerPoint.
  • Must have at least six months of verifiable experience working in public health.     
  • Must have a minimum of a bachelor’s degree from an accredited college or university in Public Health, Community Health, Health Education, or equivalent work experience.    
  • Must be willing to occasionally work evenings and/or weekends, to participate in community meetings and/or accommodate outreach activities. 
  • Must have a valid state unrestricted driver’s license.
