Position Title: Senior Community Health Nurse Position Number: 64051122

Your Specific Responsibilities:

The incumbent an employee of the Florida Department of Health in Clay County works to protect, promote and improve the health of all people in Florida through integrated state, county, and community efforts.   The incumbent is expected to promote innovation by searching for creative solutions and managing resources wisely.  They are expected to collaborate through teamwork to achieve common goals and solve problems.  The incumbent is expected to be accountable and perform with integrity and respect. Program Oversight:

  • Visits assigned schools to ensure adequate program oversight.
  • Performs fall and spring School Health Services Program Reviews and Focus Visits of each assigned school health room using the current review form.
  • Initiates process improvement plans and follow-up visits, at assigned schools, as needed, for schools not meeting the performance measures during the fall and/or spring School Health Services Program Reviews, during a school visit or for non-compliance record reviews. 
  • Reports School Health Services Program Review findings to appropriate school personnel.
  • Provides annual updates on School Health Services Manual and other reports, as assigned, verifying information for accuracy, completeness, correct spelling, punctuation, and grammar usage.
  • Updates and creates internal forms for the School Health Services Program, as assigned by supervisor.
  • Provides consultative and technical guidance to the Clay County District Schools (CCDS), as requested, for the development of their forms.
  • Assists with collecting, analyzing, and inputting of data/statistics for School Health Services Annual Report, School Health Services Plan, school district monthly reports, mandatory mass health screening data, annual kindergarten and seventh grade immunization reports, end of the year reporting or any other report, as assigned.
  • Updates information in the sub-folders that are part of the School Health Services Manual and/or are used by the School Health Services Program. 
  • Conduct school visits, attend community meetings or community outreaches utilizing a county vehicle or own vehicle, as per policy.

 

Health Screenings:

  • Supports CCDS in completing health screenings, as available.
  • Conducts health screen training for school nurses, DOH staff and school volunteers.
  • Ensures that school health designee completes rescreens and screening of absentees, per state designated time frame.
  • Follows up with school health designee regarding outcome of referrals. 
  • Develops screening follow up letters and forwards to the CCDS for distribution.
  • Participates in screenings, per Memorandum of Agreement (MOA), for private schools in Clay County.
  • Codes and inputs health screening results and outcomes into the Health Management System by designated timelines, as assigned.

Trainings:

  • Train registrars, and school health designees on current immunization and school entry health exam requirements.
  • Conducts 4-hour orientation class for new school health designees.
  • Performs child specific training with unlicensed assistive personnel (UAP) in schools without a registered nurse, as per the MOA.
  • Attends in-service workshops, seminars, and staff conferences to update professional knowledge and skills.
  • Serves as preceptor for new employees and conducts assigned trainings for DOH staff and school staff.

 

Collaboration:

  • Effectively communicates with CCDS school health designees (RN, LPN, Aide), principals, social workers, registrars, guidance counselors, bus drivers, and any other school staff.
  • Collaborates with the schools to evaluate student specific health care needs and coordinate care between school staff, parents, and healthcare providers to assure a healthy learning environment for the students.
  • Provides, organizes, and participates in outreach events with community partners.
  • Conducts school health entry requirement (school physical, immunizations) record reviews, as needed, in CCDS.
  • Assists DOH-Clay Epidemiology Program with outbreak investigations, upon request.
  • Collaborates with community partners by attending and hosting meetings.

 

Meetings:

  • Active member of the School Health Wellness Advisory Committee (SHWAC) and community committees, as assigned.
  • Updates the school health team regarding community committees.
  • Attends 504 and IEP meetings, in schools without a registered nurse, as per MOA.
  • Participates in, develops trainings, and/or provides trainings for the school health services annual meetings.
  • Attends and participates in DOH meetings, school health monthly staff meetings and any other meeting and committees, as assigned.

 

Other duties and responsibilities:

 

  • Resource to school health staff regarding health education, by providing health education material.
  • Creates care plans and emergency action plans for students at schools that do not have a registered nurse, as per MOA.
  • Completes records, reports, and coding for continuity of school health services.
  • Data entry of school health room activity reports from previous month into the Health Management System (HMS) by the 15th of the current month.
  • Communicates with parents and other professionals by telephone, home visits, or letter for follow-up of health problems or continuity of care.
  • Responsible for securing the designated information sets for protecting confidentiality, data integrity and the appropriate access for all information, both confidential and public record, stored in hard copy or electronic format.
  • Provides nursing services for immunization program during back-to-school events or as assigned.           
  • Completes certified DH 680 Form / DH 681 Form for immunization program, as assigned, and requested.
  • Provides administrative support to the school health team, as needed.
  • Prepares, types, edits, and transcribes correspondence, memorandums, reports, statistics, grants, records, and other documents.
  • Schedules meeting place, time, and needed support and/or equipment for conferences, in-service trainings, meetings.
  • Assists in the preparation of presentation materials and agendas which includes sending notifications of upcoming meetings to appropriate participants.
  • Takes notes during staff meetings to draft minutes and distribute to participants.
  • Responsible for collecting and input of statistical information sent from the schools into the appropriate data base or document.
  • Assists with grant preparation and maintenance, by providing administrative support, as directed.
  • Takes active role in participating in quality improvement initiatives, including team lead.
  • Assists with budgets by providing administrative support, as directed.
  • Creates and maintains budgetary spread sheets, inputting and tracking of expenditures, and assisting with ensuring proper expenditure code use.
  • Prepare/create purchase requisitions for school health purchases using Ariba/ MFMP. 
  • Maintains all documents needed to complete purchasing following DOH purchasing policies and procedures. 
  • Reviews and ensures that orders are correct, and received as required, to complete the process, including scanning appropriate documentation.
  • Update and create school health services contracts, as assigned.
  • Provides and meets deliverables of any school health services contracts, as assigned.
  • Serves as Records Management Liaison / Coordinator for the School Health Office in accordance with DOH Retention Schedules, the General Records Schedule GS1-SL, and form DH1305.
  • Performs other related duties as required/assigned and within the realm of licensure, protocols, and the classification specifications of a Senior Community Health Nurse.
  • Performs all the other duties as required.

  • Work related experience with Florida Shots and school entry immunizations preferred.
  • BSN preferred from an accredited school of nursing.
  • Work related pediatric experience preferred.
  • Prior school health experience preferred.
  • Bilingual preferred.

Qualifications:

Minimum –

  • Must be required to have access to a privately owned vehicle to perform duties if a state vehicle is not available to attend trainings, and to travel to outlying schools or facilities. You will be reimbursed for this travel at the state approved rate.
  • Must be proficient in the use of the following Microsoft Office applications: Word, Outlook, Excel, and PowerPoint.
  • Must have the ability to present trainings and presentations in front of large groups.
  • Must be licensed as a Registered Nurse in the state of Florida.
  • Must possess a valid State Driver's License.
