Your Specific Responsibilities:

The incumbent is part of a global public health organization and is expected to support the Florida Department of Health in Clay County in its mission to protect, promote and improve the health of all people in Florida through integrated state, county and community efforts and its vision to be the Healthiest State in the Nation.  Furthermore, the incumbent is expected to incorporate the agency values of Innovation, Collaboration, Accountability, Responsiveness and Excellence in all aspects of their work.

This is a highly responsible position in the Ryan White program at the Florida Department of Health in Clay County. The incumbent serves as eligibility worker and ADAP case manager ensuring all clients have access to clinical and support services.  Incumbent works in partnership with private and public partner agencies and stakeholders, and other HIV/AIDS Service Organizations including Community based organizations. 

Responsible for identifying clients for core Ryan White (RW) eligibility and overseeing the AIDS Drug Assistance Program (ADAP). Understands and uses HMS, CAREWARE, and Provide Enterprise (ADAP) for data input.

Staff will accurately and timely complete and document all client encounters in HMS, State, RWHAP systems (Provide), and any other system as determined by the program. Staff will complete financial eligibility determination in accordance with State of Florida requirements for clients of DOH-Clay and other eligible clients for enrollment to the Ryan White and ADAP programs.

Staff will ensure that all clients are screened for other payor sources including, but not limited to, Medicaid, Medicare, or other available public or private programs at the time of eligibility enrollment and re-enrollment.  Staff will verify income, residence, clinical status, recertify, and unenroll appropriate clients in time frames as specified by HRSA, RWHAP, and State of Florida standards and per those protocols set forth.

Staff will contact clients regarding social service referrals, updated insurance status, eligibility renewals and determination, and eligibility appointment reminders. Contact may be face-to-face, by telephone, mail, or home visits using county car as per client and program needs.

Staff must maintain subject matter expertise for assigned duties and participate in all training activities and attend meetings as required by the program may have to travel using county vehicle.

This position will serve as a backup to the medical case manager for the Ryan White program at the Florida Department of Health in Clay County. Assists with medical case management functions including but not limited to initial assessments of needed services, completion of needs assessments, plans of care, client linkage and referral to necessary medical services. All services provided through office, phone calls, and /or field visits using county vehicle.

Represent FDOH-Clay at Ryan White and HIV/AIDs events in the area and participate in community meetings using county car, if available to travel.  Conducts outreach education with local providers and clients to bring known and unknown HIV positive individuals to care and to assist in the continuity of care for clients.

Required Knowledge, Skills, and Abilities:

  • Knowledge of Microsoft applications.
  • Knowledge of State of Florida Personnel Rules and applicable Florida Statutes.
  • Ability to communicate effectively both in writing and orally.
  • Ability to prepare accurate reports timely.

Qualifications:

Minimum –

  • Must have a bachelor’s degree in psychology, social work, or human services; a bachelor’s degree in any field of study with at least six months of case management; or a registered nursing license in Florida.
  • Must have any experience with Sexually Communicable Diseases investigations, including HIV.
  • Must have a valid State Driver's License or are able to attain one within 30 days of employment.

Preferred –

  • Completed the 500/501 HIV Training in the last 12 months.
  • Six months of case management experience.
