This is a Career Service Senior Human Services Program Specialist position working for the Lee County Health Department.

 

The incumbent in this highly responsible leadership position will be responsible for providing technical assistance to the designated county health departments (CHDs) in Area 8 with regards to control and prevention of tuberculosis (TB).  Review of each case of TB and associated contacts in the Area is the major expected deliverable.  He/she directly contributes to the Florida Department of Health (DOH) objective of reducing the rate of infectious diseases.  The position is assigned and administratively supervised by the Lee CHD.  The position’s performance evaluation will be conducted in collaboration with all the CHDs in the Area and the Bureau of TB and Refugee Health (BTBRH) in Tallahassee.  The position—as a shared resource between CHDs in Area 8—requires strong case management skills, ability to work independently, ability to manage multiple program and policy issues simultaneously, and excellent communication skills.

 

Surveillance Activities – Incumbent is responsible to ensure the timeliness, accuracy and completeness of TBrelated data contained in the Florida Department of Health’s (FDOH) Health Management System (HMS), the Centers for Disease Control and Prevention’s (CDC) Report of a Verified Case of Tuberculosis (RVCT) and the CDC Electronic Disease Notification (EDN) Reporting System in the assigned consortia area(s).  These duties will be achieved by:

  • Completing and/or conducting a data quality assurance review of all RVCTs and EDN reports upon submission from the CHDs, rejecting submissions as necessary and communicating/educating the reason(s) for rejection to the CHD staff member assigned responsibility for completing the form locally.
  • Working with appropriate CHD staff members to address issues identified by the “alerts” generated by the TB Control Section in a timely manner.
  • Utilizing tools available on the restricted TB Control Section’s SharePoint site to identify and resolve incomplete reporting or documentation by the CHD, assess and share reasons for negative outcomes with CHD and provide periodic CHDspecific performance reports, as requested or required.
  • Submitting High Profile Notification Forms to the TB Control Section as required and within defined timelines.
  • Generation of countyspecific demographic and performance reports as requested or warranted by CHD Directors and/or Administrators,
  • Completing surveillance reports in STARS, as applicable.

 

Technical and Onsite Assistance/Mentoring – Incumbent will provide technical assistance as requested and warranted to local CHD staff responsible for the duties listed below in the assigned consortia area(s).  This assistance will be with, but not limited to, the following:

  • Contact investigations,
  • Lost and delinquent investigations (return to or initial care),
  • Documentation in HMS,
  • Surveillance reporting (RVCT/EDN),
  • Interjurisdictional Notifications (incoming and outgoing),
  • Acquiring incentives and enablers,
  • Identifying potential providers of temporary housing or inspection of identified housing for appropriateness,
  • Recommendations for resolution of identified TB program related issues,
  • Community partner outreach,
  • Orienting and/or mentoring new or existing staff in all topics listed above.

 

Program Evaluation/Quality Improvement Activities – Incumbent will serve as a member of a multidisciplinary team to provide these services to the CHD.  Duties will include, but not be limited to:

  • Preevaluation performance assessment,
  • Staff interviews and shadowing,
  • Medical record auditing,
  • Assistance in implementation of evaluation recommendations,
  • Participation in TB case and cohort reviews,
  • Provision of CHDspecific performance feedback,
  • Provision of data analysis and interpretation for program performance improvement and/or efficiencies,
  • Provision of onsite information gathering at state and federal correctional facilities related to TB suspects, cases and contact investigations in these institutions.

 

Compilation of quarterly activity reports providing a summation of surveillance, technical assistance and program evaluation/quality improvement activities in the previous quarter throughout the assigned consortia area(s) and submission to CHD Directors or Administrators and the TB Control Section.

 

Information Custodian: This position has the responsibility of being the Information Custodian for the TB Surveillance information set and, will perform related duties as defined in DOH 50-10.

  • Selected immunizations or titers and/or periodic screening for tuberculosis may be recommended for this position.
  • Incumbent must possess a valid Florida Driver License and have a motor vehicle to carry out these duties.

  • Transporting Confidential Information: This position is authorized to transport confidential information outside of established DOH-Lee facilities and must comply with parameters defined in DOHP 50-10.
