Your Specific Responsibilities:
The Disease Intervention Specialist (DIS) is a professional public health position responsible for conducting confidential STI and HIV case investigations, providing partner services, performing outreach and testing activities, and ensuring appropriate linkage to care in accordance with Bureau of STD Prevention and Control guidelines. The incumbent works within the STD Program to detect, control, and prevent the spread of sexually transmitted diseases, including chlamydia, gonorrhea, syphilis, and HIV, throughout Area 15, which includes St. Lucie, Martin, Okeechobee, and Indian River Counties. The position operates under the direct supervision of the Public Health Services Manager–SES and is classified as a sensitive position due to regular access to protected health information. The incumbent handles confidential client information strictly on a need‑to‑know basis and must follow all Department of Health Information Security and Privacy policies, protocols, and procedures. Employees are required to comply with all state and federal laws governing the confidentiality, security, privacy, and integrity of protected health information. All client information used or obtained in the field must be securely maintained and protected from access by unauthorized personnel.
Case Investigation, Partner Services & Linkage to Care
The DIS manages a personal caseload of STI and HIV investigations using the STARs database and follows established state and local protocols. The incumbent conducts interviews, re‑interviews, record searches, telephone investigations, and field visits to locate infected individuals, notify them of laboratory results, elicit partners, and offer partner services. The position requires advanced understanding of signs, symptoms, transmission, and disease progression of STIs and HIV to properly stage infections and provide accurate education and counseling. The DIS interviews individuals diagnosed with STIs, including HIV/AIDS, to educate them about their infection, guide them through risk‑reduction strategies, support future medical follow‑up, and develop plans to locate and refer sexual partners or others who may be at risk. The DIS ensures that all infected individuals and their identified partners are referred promptly to appropriate medical facilities for examination and treatment. This may include clients located in jails, rehabilitation centers, clinics, hospitals, private provider offices, and various community settings such as homes, parks, or homeless encampments. The position requires the use of county or state vehicles to conduct field investigations and, when needed, to transport clients to and from medical appointments in accordance with local and state policies governing vehicle use. Investigation may also include conducting internet searches and communication using select mobile dating applications approved by the STD program office to notify people exposed to or at risk for STDs including HIV. Ensures that HIV clients are referred to Test & Treat appointments, Linkage to Care, Ryan White, and Maternal Child Health programs in accordance with the guidelines and timelines for each. All partner notification activities will be carried out within established communication parameters as outlined in the internal operating policy, STD and HIV Partner Notification via Mobile Dating Applications. Consults with providers, infection control, laboratories and other community partners as needed for: case reporting, obtaining information on cases or determining diagnosis, provide current guidance on diagnosis and treatment. Works closely with and is cross trained as a Linkage to Care Coordinator to identify newly diagnosed clients and re-engage clients who have fallen out of care and assist providers in locating non-compliant clients and bring them back into care & treatment. Identify potential barriers to care and complete necessary referrals. Documents initial and periodic reviews of interview, counseling and investigative forms.
Field Testing, Outreach, & Community Education
The DIS conducts targeted field‑based STD and HIV screenings, including serological testing, blood draws, and urine collection, while adhering to phlebotomy standards and universal precautions. The incumbent participates in planning, implementing, and evaluating community outreach and health education activities aimed at reducing transmission and improving community health outcomes. Responsibilities include coordinating logistics such as supply management, transportation, and event setup and breakdown. The position regularly communicates with providers and laboratories on reporting requirements as appropriate, provides education including CDC guidelines for testing, preventative treatment, and treatment of STDs in accordance with Florida Statutes 381.0031, 381.004, and 384.24. The DIS initiates and participates in provider visits and delivers in-service education on STD/HIV testing and treatment, ensuring that providers understand and follow statutory requirements under Florida Statutes 381.0031, 381.004, and 384.24.
Training, Documentation & Administrative Responsibilities
The DIS assists with training new staff, including field shadowing, STARs documentation, interviewing techniques, and partner services protocols. The incumbent participates in required meetings, program trainings, chalk talks, conference calls, and program planning activities. The DIS ensures that all assigned cases are documented and closed according to statewide standards and contributes to program evaluation and performance improvement efforts. Required administrative tasks include completing travel reimbursements, HMC service coding, Employee Activity Reports (EARs), and reporting outreach data to supervisors.
Performs additional responsibilities as assigned, which may include supporting outreach activities, assisting with special projects, and providing backup coverage for other team members and the STD Program Manager in their absence or when needed. The incumbent maintains strict client confidentiality and protects all sensitive information in accordance with DOHP50-10-10 Information Security and Privacy Policy for Confidential Information. All protected health information must be safeguarded at all times from access by unauthorized personnel.
Required Knowledge, Skills, and Abilities:
Must be able to travel in and out of county within area 15must have a valid driver’s license, case manager experience, phlebotomy training. Self-motivated, self-starter willing to take initiative, flexible, ability in PowerPoint presentations skills, exceptional interpersonal skills, efficient in communication, good organizational and time management skills, ability to work in teams, Able to complete Linkage to Care (ARTAS) and Disease Intervention Specialist training, experience with delegating tasks, must be comfortable with talking to individuals and groups. Knowledge and experience in working with diverse communities. Ability to plan and coordinate work assignments. Experienced using Microsoft Office (Word, PowerPoint, Excel, Outlook). Must be able to lift and/or move up to 50 pounds, reach, stop climb ladder and bend.
Qualifications:
Minimum:
- Must have a valid driver’s license.
- Must have minimum of one year experience working in an HIV/STD/Hepatitis Prevention, Linkage, or Case Management Program.
- Must have minimum of one year experience working as lead team and/or program.
- Must be willing to serve in special needs shelter during hurricane season and/or any other emergency duties involving natural and/or man-made emergency.
- Must be willing to travel out of county, into varied socioeconomic neighborhoods, out of county travel, 25-50% of time per week.
Preferred:
- Experience managing STI/HIV caseloads and/or daily cases investigation.
- Experience managing HIV/STD/Hepatitis Prevention, Linkage, or Case Management and/or related work.
- Associate degree or equivalent relevant work experience in a healthcare or human services field preferred.
- Certified phlebotomist and/or trained phlebotomist.
- Bilingual Spanish and/or Creole preferred
- Experience in a Lead Role
