Your Specific Responsibilities:
This position will service the AREA 15(Indian River, Martin, Okeechobee, and St. Lucie Counties)
Ryan White Patient Care Programs as medical support Services Specialist (MSS).
The incumbent will be the Key point of Contact for all specialty care and support service referrals on behalf of a client made by subcontracted and providers and case managers; Telephone and in-person reception and direction of clients.
New and re-enrollment client intake, eligibility determinations and eligibility verification. In instances of extended absences, vacancies or need for general coverage, position will serve as a back-up to medical case managers assisting with completion of needs assessments, plans of care, client linkage and referral to necessary services.
Services provided through in office and field visits. This position reports to the Operations Services Manager.
Care Coordination
Receives via fax or hand delivery, medical care and support service referrals from providers and case managers on behalf of eligible Ryan White Part B clients; Verifies eligibility, insurance and whether referral request is HIV related; Coordinates the client’s attendance for appointment with referring provider, other care coordinators and assigned case manager. Referrals processed within 30 business days from date of receipt. Documents barriers for unmet care coordination timeline. Enters data on referral within electronic reporting databases including CAREWare, HMS and other data systems where mandated within 3 business days or before of referral process.
Ryan White Customer Service
Serves as receptionist to the main case management office located in St. Lucie County including telephone and in person reception. Provides requested information to clients and service providers including eligibility verifications. Responsible for mail, fax and office supply distribution.
Ryan White Enrollment and Eligibility
Processes new and re-enrolling request for client intake into Ryan White program services. Screens for insurance, medicaid and medicare eligibility utilizing required information systems Availity, Change Healthcare and others. Screens clients to determine if client has access to or can obtain private health insurance and/or VA benefits. Request for enrollment and eligibility processed within 14 business days from date of referral or pre-enrollment screening. Documents barriers for unmet enrollment time. Enters enrollment and eligibility case notes in CAREWare, HMS and other data systems where mandated within 3 business days or before of direct client encounter or action taken on behalf of client. Networks with other departments including but not limited to Ryan White case management, HOPWA, prevention, DIS, TOPWA, HUG Me and other participating providers to assure smooth transition of services and continuity of care.
Administrative
Participates in conference calls, planning meetings regarding Ryan White Patient Care and other departmental meetings when necessary. Completes DOH and Ryan White mandatory trainings in person and via webinars. Completes EARS/CSR, timesheets, program reports within required timeframes. Submits within required timeframes but not limited to cell phone verifications, travel documentation, invoices other required reports etc.
Required Knowledge, Skills, and Abilities:
Knowledge of techniques for effectively dealing with people; Knowledge of techniques for effective communication; Ability to prepare correspondence and administrative reports; Ability to work independently or with minimal supervision; Ability to plan, organize and coordinate work assignments; Ability to communicate effectively verbally and in writing; Ability to prioritize individual work load; Must be very detailed oriented and capable of thinking independently while exercising
good judgment. Required to make independent judgments without supervision; Must be able to perform under stress when confronted with emergency, critical, or unusual situations; Must be capable of dealing with periodic cyclical workload pressures and levels of responsibility.
Qualifications:
Minimum:
- Three years Documented year experience in customer service (telephone etiquette, face to face contact with public)
- Three years’ experience in intake/ assessing clients/patients (ex. -medical history; social history)
- Florida Driver’s License or willing to get one within 30 days of hire.
- Three years of medical referrals and patient care follow up.
- Bilingual - English/Creole or English/Spanish.
