 Your Specific Responsibilities:

This incumbent shall be responsible for determining an individual’s eligibility for the AIDS Drug Assistance Program (ADAP) and provide direct Case Management and other ADAP services in accordance with State and Federal Guidelines and Policies of the ADAP Program within St. Lucie, Indian River, and Okeechobee Counties. Clients enrolled in ADAP must first be deemed eligible through the Ryan White Medical Case Management Program.

 Duties include but not limited to; Determining Eligibility for ADAP and ADAP Premium Plus; Receiving and distributing Pharmaceutical Prescriptions; Entering, updating, and Maintaining ADAP Client records within the State ADAP electronic records database in addition to the Electronic Health Records System of the Florida Department of Health in St. Lucie County as well as other mandated EHR systems.  In addition, the incumbent will serve as needed as back-up to the Ryan White Case Management program assisting in front office reception and medical case management job functions including but not limited to eligibility determinations, completion of needs assessments plans of care, client linkage and referral to necessary services. All services are provided through office or field visits.

 

ADAP Eligibility, ADAP Case Management, Counseling, Education, Medication Supply/Distribution/Inventory

Provides ADAP Program Services including but not limited to:  Determining ADAP Eligibility, Case Management, treatment adherence counseling, medication ordering and distribution, medication inventory. Referral for enrollment processed within 7 business days after date of receipt. Documenting difficulties barring enrollment.   Ensuring completion of all Affordable Care Act documentation within required timeframes; Client’s eligibility re-determinations are completed during the fifth month or at the least two weeks prior to eligibility expiration. Services are provided through office or field visits.

 

Completes and maintains accurate documentation of all client-specific activities.

Responsible for accurate case note documentation and entry of required data fields into various electronic reporting systems directly related to job duties including but not limited to Provide, CAREWare and the Health Management System for the State of Florida Department of Health. Case notes to include but not limited to medication changes; adherence issues; reminders; discussions with assigned RW case manager, providers; insurance companies and the like. Enters case notes documentation, required enrollment and programmatic form scans within 3 business days of direct client encounters or any actions taken on behalf of a client.

Client Support and Follow Up/Adherence

Provides follow up with clients who are non-compliant with eligibility re-determinations and medication pickups. Notifies assigned RW case manager of first missed appointment in either instance. Documents efforts of follow up including but not limited to Provide, CAREWare and the Health Management System for the State of Florida Department of Health. Participate in case conferences when necessary. Networks with other departments including but not limited to Ryan White case management, HOPWA, Prevention, DIS, TOPWA, HUG Me and other participating providers to assure smooth transition of services and continuity of care.

Administrative

Submit ADAP specific reports as required.

Participates in ADAP and all other conference calls, planning meetings regarding ADAP and Ryan White Case Management duties and other auxiliary services and other departmental meetings when necessary. Completes DOH, ADAP and Ryan White Case Management mandatory trainings/webinars. Completes EARS/CSRS and timesheets within required timeframes. Submits within required timeframes but not limited to cell phone verifications, travel documentation and invoices etc. Responsible for medication ordering, distribution, and medication inventory

Ryan White Case Management Program Support

Provides back up coverage for the Ryan White case management program assisting in front office reception including telephone and in-person reception, mail, and supply distribution; Provides essential information to clients and service providers including eligibility verifications. Provides medical case management job functions including but not limited to processing new and re-enrolling client intake applications for eligibility re-determinations, completion of needs assessments and care plans.

Qualifications:

 

Minimum:

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
  • Associate degree or higher from an accredited college or university in Social Work, Human Services, Psychology, or Sociology or Health Sciences.  
  • Two years documented professional work experience at a social service-related agency or other social services work position.

 

Preferred:

  • Bilingual (English/Spanish) / (English/Creole)
  • Tri-Lingual (English/Creole/Spanish)
