Your Specific Responsibilities:

The incumbent acts as the Regional Linkage Re-engagement Coordinator (LRC) and is responsible for the program coordination of Area 15 HIV/AIDS Linkage to Care programs and staff, ensuring individuals newly diagnosed with HIV, or re-engaging in care are quickly connected to medical and supportive services. This position supervises Linkage to Care Coordinators and Jail Linkage programs.  Facilitates and supports community planning and mobilization activities; Provides and assists with HIV/STD/Hepatitis counseling testing with the current preferred method (finger stick for rapid and venipuncture (blood draw) for confirmatory test on all positives) and educational services to clients and staff in St. Lucie County/Area 15. This position will work in collaboration with the HAPC, HIV Lead Agency/QA team Manager, HIV/STD Program Manager and Patient care manager. This role combines direct client coordination with team supervisions, overseeing linkage specialists along the continuum of care. Must be Phlebotomy and Disease Intervention Specialist trained.  Works under the direct supervisor of the Public Health Services Manager-B. Must be willing to travel out of county and/or other areas. Must be willing to work before and/or after-hours including weekends. This position and will be expected to support integration efforts to enhance disease control programs capabilities.

 

Supervision/Program Management

Supervises day-to-day operations of the HIV Linkage to Care and Jail Linkage programs and staff responsible for serving St Lucie County or Area 15 (Indian River, Martin, Okeechobee, and St Lucie County). This position will be expected to cross train and support integration efforts to enhance disease control programs capabilities and ensure coverage. Analyzes program outcomes on an ongoing basis for effectiveness in meeting stated goals and objectives. Monitors and evaluates workloads, workflow, deadlines, and work productivity. Provides consultation and makes appropriate recommendations for action. This includes conducting performance appraisals, approving leave, hiring, and taking disciplinary action or effectively recommending such actions. Plans, organizes, directs, and coordinates the activities of the HIV Linkage program. Coordinates and ensures local and Area 15 HIV Linkage and Jail Linkage services are available. Ensures relevant training is provided as needed and educational materials are distributed through Area 15. Prepares and maintains timely and accurate records including work plans and submits all necessary reports on time according to program guidelines and requirements. Keeps Supervisor and other stakeholders routinely informed of changes noted or issues of concern. Maintains relationship and regular communication with Area 15 HIV/AIDS points of contact and their direct supervisor/administrators. 

 

LRC Duties/Testing Program/TA

Act as the lead linkage coordinator point of contact for Area 15 counties. Assist Prevention Program Manager to provide oversight to three Linkage to Care (LTC) staff. Coordinate with and act as a liaison to linkage specialists in Area 15 to ensure Linkage to Care (LTC) field records in the HIV/AIDS Linkage Module and STARS database are followed up on and closed out in a timely manner within the STARS and Linkage Module database. Regularly monitor the status of all LTC field records for the region/county and provide technical assistance and training to linkage specialists to ensure the accuracy of linkage outcomes and timeliness of record close-outs. Ensure all newly diagnosed individuals are linked to care within 30 days. Ensures previously diagnosed are re-engaged into care within timelines. Coordinate with local HIV/AIDS surveillance and STD program staff to ensure Data-to-Care (D2C), Molecular Monitoring Project (MMP), Molecular HIV Surveillance (MHS), and Ryan White Re-engagement line lists are dispersed and worked in a timely manner and complete the D2C collection form to appropriate staff. Assist HIV-positive individuals with the practical aspects of adherence such as transportation, obtaining needed medications, receiving Medicaid or private insurance, etc. Communicate regularly with surveillance, STD and other staff to assist with linkage and re-engagement activities for the area/county. Serves as back-up to Linkage coordinators: schedule initial appointment(s) for HIV positive clients for access into the medical care system for these individuals, complete Ryan White Patient Care Eligibility Application when necessary and set up. Enter all testing, linkage and/or re-engagement services into the HIV/AIDS Linkage Module, Health Management System (HMS), the State Ryan White CAREWare system, the STD STARS system and all other reporting systems as needed on a regular basis. Follow up with the provider and individual to confirm that the individual kept their set appointment. Attend client initial linkage and/or re-engagement appointments, as necessary. Collaborate and coordinate with HIV testing, linkage and Disease Intervention Specialist (DIS) staff at County Health Departments (CHD), public and private medical providers, HIV case managers, community-based organizations (CBO), Targeted Outreach for Pregnant Women Act (TOPWA) providers, perinatal linkage staff, State and County correctional staff, community health center, and other HIV testing sites to link HIV-positive individuals into the HIV/AIDS system of care throughout the area/county. Have knowledge of local care and treatment delivery system. Enter all testing, linkage and/or re-engagement services into the Health Management System (HMS), the State Ryan White CAREWare system, the STD STARS system and all other reporting systems as needed on a regular basis. Provide HIV education and information to HIV-positive individuals as needed. Participates in clinic, community or impromptu screening activities by performing venipuncture or using other rapid testing techniques to identify HIV/AIDS. Provides outreach counseling and testing and HIV/AIDS education presentations at community events including health fairs. Ensures complete and accurate processing of specimens collected.

 

Leadership/Coordination/Community Meetings

Participates in local and statewide, meetings, trainings and local committees, community, staff, and unit meetings. Assists and participates in facilitating St. Lucie County HIV Prevention Planning Group and community mobilization initiatives. Attends local community planning partnership meetings and provides TA as needed. Participate in HIV/AIDs consortia meetings.  Participates in health fairs, community screenings, and HIV educational presentations as requested by community partners. Distributes current HIV/AIDS information, educational materials, and statistics within the community. Participates in the implementation of counseling and testing, surveys, focus groups and community needs assessments. Maintains professional working relationships with other departments, agencies, and community members

 

Required Knowledge, Skills, and Abilities:

Knowledge and experience working with  HIV/STD/Hepatitis Prevention, Linkage, or Case Management Programs (minimum three years’ experience)

Knowledge of HIV disease process, treatment, prevention measures

Knowledge of Ryan White Programs and/or other HIV/AIDS Patient Care Programs

Skill as a phlebotomist or willing to become a certified phlebotomist within 6 months of hire.

Skills as a team builder and leader or supervisor (minimum two years’ experience)

Ability to communicate with variety of audiences in verbal and written forms.

Ability to work independently.

Ability to create presentations and give health education presentations and trainings to educate stakeholders in HIV/AIDS topics (minimum two years’ experience)

Ability to travel.

Must be willing to coordinate and conduct outreach and field visits in high-risk areas. Skills needed for this position include an understanding of disease surveillance systems, epidemiologic investigation techniques, and the ability to analyze data and apply disease control methods to individuals and the community. The position will utilize HMS, PRISM and other data management or reporting systems to facilitate the reporting and analysis of reportable diseases in the community.

 

Qualifications:

Minimum:

  • Associate degree or equivalent relevant work experience in a healthcare or human service field.
  • Driver’s License, if out of state, a valid Florida Driver's License is required within 30 days of hire
  • Minimum of three years’ experience working in an HIV/STD/Hepatitis Prevention, Linkage, or Case Management Program.
  • Minimum of two years’ experience in a lead or supervisory position
  • Minimum of two years’ experience giving HIV/AIDS education presentations and training.
  • Required to travel out of county, into varied socioeconomic neighborhoods, out of county travel, 25-50% of time per week.
  • Phlebotomy certification or willing to become certified within 6 months of hire. 
  • Willing to periodically work before and/or after work hours, including weekends to organize and participate in outreach events.
  • Willing to complete Anti-Retroviral Treatment and Access to Service Training (ARTAS)

 

Preferred:

  • Bachelor’s Degree or higher in a healthcare or human service field.
  • Phlebotomy certification
  • Bilingual or multilingual in Spanish and/or Haitian Creole
