Position Summary

This is a professional position of Human Services Program Specialist that works in the Community Affairs and Pre-enrollment Work Stream in the Southeast Region, APD. This position works directly with the Pre-enrollment intake unit assisting individuals applying for services through the Agency for Persons with Disabilities.

 

The Work You Will Do 

  • Arranges for the purchase of the appropriate psychological evaluation and adaptive testing for applicants if collateral information does not clearly determine eligibility. Serves as a back-up witness for the Agency in due process hearings regarding case closure appeals or eligibility for services and in other court hearings that pertain to questions regarding application process, policy required time frames and/or eligibility. Collaborates with team members and attending hearings as assigned. Requires knowledge of standard and specialized testing to determine eligibility. Responsible for organizing the intake and eligibility documentation received.

 

  • Reviews evaluations submitted for eligibility within required timeline, ensuring that they have been completed by the required professionals per the 65G-Florida Administrative Code. Ensures documentation referred for review within the unit is the required information for eligibility review/determination.

 

  • Working directly with applicants applying for services by assisting with the application of service, explaining the rule/regulations followed in regard to what type of supporting documentation is required and applicable for a particular diagnosis, to support eligibility. Scans and labels evaluations and assessments to iConnect.
  • Ensures applicants understand the age limitations for the support of a developmental disability. Works closely with applicants to be sure there is an understanding of what type of testing, evaluations or medical information is necessary to determine eligibility. Sends letters acknowledging receipt of application, request of additional information and application closure letters if supporting documentation is not received.

  • Works closely with community advocates, school system, primary care physicians, health agencies and others to obtain supporting documentation to support eligibility as well as educating this community in regard to the services and requirements to obtain services through the Agency for Persons with Disabilities.

 

  • Prepares and sends written notice of any agency action in accordance with the policy on Due Process Notification. Written notice must be sent via postal services and can also be followed up with a notification emailed.

 

  • Offers community resources and information and other duties as needed.

 

 

Minimum Qualifications

 

  • High School Diploma or its equivalent.
  • One year case management experience.
  • One year experience in customer service.
  • One year experience in reviewing psychological evaluations, individual evaluation plans, therapy reports, medical records, and school evaluations.
  • Two-year experience using Microsoft Outlook, Word, and Excel.

 

Knowledge, Skills, And Abilities

Knowledge of:

  • Knowledge of or ability to learn the Agency for Persons with Disabilities eligibility criteria.
  • Standard and specialized testing and which tests are accepted for eligibility purposes.
  • Psychological evaluations, Individual Evaluation Plans (IEP), Therapy reports, medical records and school evaluations.
  • Knowledge of case management.

Ability to:

  • Follow a procedure that requires posting and tracking to specific locations.

Demonstrate Skills in:

  • Proficient use of the computer is required, including Outlook, Excel, Access, SharePoint, and other software programs.
  • Skill in providing customer service.

Direct Deposit Program

As a condition of employment, a person appointed to a position in the State Personnel System is required to participate in the Direct Deposit Program.  Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. Retirement funds are also required to be in the Direct Deposit Program.

Background Screening Requirement

It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.

No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.
Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement and Federal criminal records checks through the Federal Bureau of Investigation and may include local criminal records checks through local law enforcement agencies.
