Your Specific Responsibilities:
This position works in the Healthy Start Program at the Okaloosa County Health Department. Position provides care coordination, case management, education and referral/follow-up to women during pregnancy and to their infants after delivery. Work is performed under the supervision of the Women, Infants & Children (WIC) Program Director, and may be performed in a variety of settings including clinic, school, and home settings. Duties are required to be carried out with strict adherence to all Healthy Start, Florida Department of Health, and Okaloosa County Health Department guidelines, policies and procedures. Personal vehicle required for use when traveling to home visits or other offsite locations as required to meet client needs. Reimbursement for eligible mileage will be issued in accordance with DOHP 56-37-22 Travel Policy.
Responsible for initial contact, care coordination, education and case management for Healthy Start clients and families in Okaloosa County. Maintains client records with utmost timeliness and accuracy. Assists in collection of statistical data and evaluation of information to assure objectives are met and standards are maintained. Provides parent education and affirmation in prenatal health, parenting skills, and infant/child health with referrals to other providers as indicated. Conducts and organizes educational classes and parent support groups as indicated.
Responsible for providing psychosocial assessments, ASQ's, Domestic Violence Screenings, followed by appropriate referrals for intervention for Healthy Start and other identified high risk clients. Will provide a structured interaction between the professional and client which assists client to identify concerns or needs by addressing social, emotional, behavioral and environmental issues that affect the client's well-being.
Responsible for development of a family support plan to meet identified needs. Responsible for the receipt of services, facilitating care and minimizing other obstacles to full participation in prenatal and infant health care. Documents all activities appropriately in the Well Families System and codes all services accurately into the Well Families System adhering to Healthy Start Standards and Guidelines.
Participates in monitoring/QI-QA activities to assure compliance with program guidelines, objectives and regulations. Participates in outreach activities that support the Healthy Start program. Organizes outreach activities for pregnant or parenting families. Attends appropriate staff meetings, conferences and training sessions to enhance job skills and knowledge. Participates in committees and work groups as requested.
Performs all other duties as assigned in support of the Agency’s mission and vision.
Knowledge, Skills and Abilities:
- Knowledge of prenatal and maternal health or other health practices
- Able to provide/conduct group meetings
- Ability to assist in planning and conducting a variety of training activities
- Ability to communicate effectively
- Ability to establish and maintain effective working relationships with others
- Ability to lift and carry 25 pounds
- Ability to teach healthcare with emphasis on childbirth education, breastfeeding, smoking cessation, and parenting.
- Work experience utilizing computer equipment and Microsoft Office applications
Qualifications:
- Valid U.S. drivers license required
- Personal vehicle or access to vehicle for home-based or offsite client visits required
- Bachelors degree in public health, health education or related field preferred
- 6 months experience working in a public health, health education, or clinical role preferred
