 BEHAVIORAL PROGRAM SPECIALIST

APD TACACHALE FACILITY 4 & 8

GAINESVILLE, FLORIDA

 

Assists residents with personal hygiene functions and ensures proper nourishment of assigned individuals, including but not limited to: providing assigned individuals a thorough bath/shower and shampoo as needed; providing nail care; dressing assigned individuals in clean, coordinated clothing that is in good repair and appropriate for the weather conditions; completing residents’ personal possessions inventory; assisting assigned individuals with toileting needs including assisting with cleaning up and changing clothing in the event of incontinence; providing assigned individuals with an opportunity to consume all meals, snacks and fluids in accordance with prescribed nutritional management profiles; providing assigned individuals with a thorough brushing of their teeth/gums as scheduled; providing assigned individuals with appropriate brushed/combed/styled hair; assisting assigned individuals with adjusting their clothing appropriately and keeping faces clean and free of food/liquids and/or nasal discharge. Ensures residents’ privacy and dignity is upheld. Maintains cleanliness and sanitation of the residents' environments, as assigned and in compliance with infection control procedures.

 

 Implements resident’s active treatment plans, including but not limited to conducting formalized, structured training sessions, implementing formal and informal behavioral reduction procedures as prescribed for assigned individuals; providing behavior supports to residents that are dually diagnosed and display intense problem behaviors, conducting formal and informal leisure and recreation activities for assigned individuals, conducting formal and informal language stimulation activities for assigned individuals; assisting assigned individuals with formal and informal vocational activities; conducting formal and informal physical and occupational therapy programs. Participates in support planning and other resident related meetings as necessary. Follows residents’ activity schedules as outlined. Records data daily as scheduled.

 

 Completes self-care/daily living skills assessments as assigned and write summaries for submission to the Qualified Intellectual Disabilities Professional for annual support plans. Develops and implements skill acquisition training programs, collects and completes training program data entry and monthly progress notes. 

 

Follows safety practices related to resident safety to include but not limited to, following incident reporting procedures; following resident accountability procedures and following residents’ Safety Plans. Utilizes appropriate first aid procedures. Notifies nursing staff or supervisor of any safety issues in a timely manner. Participates in shift change procedures communicating effectively with oncoming shift. Remains on duty at the end of the shift until proper staffing is assured for the oncoming shift. Stays to work the next shift to ensure adequate staffing to meet the needs of the residents. 

 

Maintains close observation, provides follow-up, and takes corrective action to prevent problems with health and safety of staff and residents. Reports physical plant problems.

 

Attends in-service training and other staff meetings as necessary. Attends mandatory training and/or maintains certification in Prevention and Safety Strategies, Cardiopulmonary Resuscitation and Direct Care In-service. Completes annual health screening. 

 

Performs other related or assigned duties, as necessary.

 

KNOWLEDGE, SKILLS AND ABILITIES

 

Ability to lift 40 pounds.

Knowledge of the methods of instructing and providing care to others.

Ability to understand and apply the methods and practices used in medical, psychiatric, nursing or child care or in working with developmentally disabled persons.

Ability to read.

Must be able to use computer programs to include Word, Excel and the Internet.

Must be able to make behavioral assessments.

Must be able to plan, organize and coordinate activities.

Ability to pay attention to details.

Ability to follow verbal and written instructions.

Ability to carry out instructions for implementing prescribed treatment plans including behavior modification activities.

Ability to instruct others in basic self-care skills.

Ability to instruct others in recreational activities.

Ability to provide instruction and personal guidance and care to residents.

Ability to communicate effectively verbally and in writing.

Ability to establish and maintain effective working relationships with others.

 

MINIMUM REQUIREMENT(S)

 

A valid Florida Driver’s License. (If initially hired with an out-of-state license the Florida resident must obtain a valid Florida State driver’s license within 30-days of hire. License suspended or revoked for any reason, work permits (Business purpose/ Employment/Education only license) and some types of restricted license are not acceptable. License that have “Corrective Lenses Restricted,” are acceptable, provided the driver wears the corrective lenses while operating the vehicle.

 

PREFERRED REQUIREMENTS

 

An Associate’s degree in a health field, behavioral or rehabilitative sciences or; two years’ experience providing care and treatment for people with intellectual disabilities and; proficient in Microsoft word.

 

WORK SCHEDULE

 

This position is located at a 24- hour facility.  Employees must be agreeable to working a shift schedule; regular, evening, or night.

 

Employees may be required to work additional days/hours based upon the needs of the home and center.  This includes, but is not limited to:  ensuring home coverage, activities, holidays, and emergency situations.
