HUMAN SERVICES SENIOR SUPERVISOR

 This position supervises and coordinates the duties and responsibilities of direct care staff on the assigned shift.  Communicates with and instructs direct care staff, through routine meetings, in-service training; staff schedules, protocols and procedures, personnel actions, work expectations and effective delivery of resident directed services.  Ensures direct care staff follow safety protocols and procedures related to employee and resident safety.  Ensures direct care staff follow infection control procedures. Manages staffing schedules and leave requirements and performs all shift change procedures to include maintaining mandatory, adequate staffing.  Tracks and provides feedback related to employee performance.  Ensures accurate timekeeping to include reviewing timesheets for accuracy and timely approval in People First.  Completes and submits monitoring reports, logs and data recording per policy and procedures and as directed.

 

Ensures that all active treatment programs, formal and in-formal, as prescribed by the Interdisciplinary Team (IDT) are implemented correctly and in a timely manner in compliance with state, federal and Medicaid regulations. Through direct observation and oversight, ensures direct care staff provide transportation of residents to the off-home programs on center in line with the residents’ daily activity schedules. Monitors the implementation of formal and informal programs and procedures for correct implementation and data collection.  Completes and maintains all required monitoring documentation in a correct and timely manner in accordance with assigned deadlines.  Ensures direct care staff stay current with information regarding the residents.

 

Assists with direct care duties including but not limited to direct accountability of an assigned group of residents, assists residents with self-care/daily living routines and leisure activities, accompanies residents to medical appointments, worksites, alternative leisure programs and therapies.  Implements behavioral supports as outlined in residents’ Behavior Support Plans and informal procedures. Interacts with direct care staff daily to create and maintain a sense of teamwork. Takes necessary action to correct any problems.

 

Conducts inspections and monitors all areas of the physical plant, ensures proper maintenance and cleanliness of the living areas and ensures the completion and follow up of corrective actions for deficiencies identified.  Prepares and submits reports per facility guidelines

 

Attends and participates in residents’ Individual Support Plan Meetings and monthly IDT Meetings, Psychiatric Clinics, and other facility meetings as assigned. Confers with the treatment team leader and home manager and/or other IDT members concerning the well-being of the residents. Works closely with off home providers specifically Behavioral Specialist, Dietitian, Occupational Therapy, Physical Therapy and Adaptive Devices to ensure that maximum care is available to the residents.

 

Coordinates and tracks direct reports’ attendance of mandatory trainings and/or certification in Prevention and Safety Strategies (PASS), Cardiopulmonary Resuscitation and Direct Care In-service.  Coordinates and tracks direct reports’ completion of their annual health screening.

 

Maintains annual training for and actual use of PASS to address residents who display aggression, self-injury, or property destruction and require interventions using a combination of verbal and physical techniques to address problem behaviors.  Lifts up to forty pounds.  Lifting is required to ensure provision of physical and nutritional management.  Maintains certification for Cardiopulmonary Resuscitation.  Attends annual Direct Care In-service. 

 

Other related duties as assigned.

Ability to establish and maintain effective working relationships with others.

 

MINIMUM REQUIREMENT(S)

 

Maintain Advanced Prevention and Safety Strategies [PASS] Certification.

One (1) year supervisory experience.

Two (2) years direct care experience.

 

A valid Florida Driver’s License. (If initially hired with an out-of-state license the Florida resident must obtain a valid Florida State driver’s license within 30-days of hire. License suspended or revoked for any reason, work permits (Business purpose/ Employment/Education only license) and some types of restricted license are not acceptable. License that have “Corrective Lenses Restricted,” are acceptable, provided the driver wears the corrective lenses while operating the vehicle.

 

WORK SCHEDULE

 

This position is located at a 24- hour facility.  Working Hours: Vary. Days Off: Vary.
