Promotes residents’ skill acquisition by developing, conducting, supervising, and participating in the overall active treatment plan of the residents assigned to the facility through a variety of therapy programs. These include social and recreational activities, leisure activities, basic academic development activities, human growth and development, motor activities, prevocational and work-related activities, self-care and daily living skills training. Trains, in-services, and monitors staff in the delivery of resident-directed services and in preparation of related written programs and reports. Maintains compliance with state, federal and Medicaid regulations and facility protocols and procedures for implementation and oversight of residents’ daily active treatment.

 

Evaluates through direct observation, direct interactions, assessments and record review to develop recommendations regarding residents’ programmatic needs. Prepares reports for residents’ Individual Support Plans (ISP) and monthly progress notes related to the residents’ ISP goals and objectives. Prepares other reports as assigned. Develops tracking systems and complies with facility deadlines. Participates in the ISP meetings, Interdisciplinary Team meetings, and other meetings as they relate to assigned resident caseload. Monitors each resident’s progress, evaluates if individual resident programs are effective, and takes necessary action to promote progress and maintenance of skill acquisition.

 

Coordinates, plans and attends/participates in special activities for residents. Is flexible with schedule regarding attending activities outside of regular work schedule, to include days off. Maintains open communication with home managers to include but not limited to posting activity calendars and activity flyers. Ensures that the required resources for these events are in place by requesting and managing the necessary funds and providing and/or assisting with transportation of the individuals to and from programs and activities. Develops monthly special activity calendars and daily activity schedules.

 

Purchases, develops, repairs (or submit repair requests), and maintains materials such as leisure/recreational equipment and instructional materials for residents as assigned.

 

Attends mandatory training and maintains certification in Prevention and Safety Strategies, Cardiopulmonary Resuscitation and Direct Care In-Service. Completes annual health screening.

 

Performs other work-related duties as required.

 

KNOWLEDGE, SKILLS, AND ABILITIES

Knowledge of Developmental Disabilities or Mental Illness, or the principles and practices of one of the allied health field principles or one of the rehabilitative sciences

Knowledge of computer applications.

Ability to communicate effectively, verbally and in writing.

Ability to establish and maintain effective working relationships with others.

Ability to plan, organize and coordinate work assignments.

Ability to plan/organize/organize/implement leisure and recreational activities.

Ability to assess and determine training needs and develop training programs in the areas of leisure/recreation and/or self-care and daily living.

Ability to write evaluation and progress reports.

 

MINIMUM REQUIREMENT(S)

A valid Florida Driver’s License. (If initially hired with an out-of-state license the Florida resident must obtain a valid Florida State driver’s license within 30-days of hire. License suspended or revoked for any reason, work permits (Business purpose/Employment/Education only license) and some types of restricted license are not acceptable. License that have “Corrective Lenses Restricted,” are acceptable, provided the driver wears the corrective lenses while operating the vehicle.

 

PREFERRED EXPERIENCE

A Bachelor’s degree or higher in a health field, behavioral or rehabilitative sciences, music therapy/education, physical education or therapeutic recreation. Allied health field, Criminology, Human Science Degree, Health Science.

 

Substitution: Four years’ experience providing care and treatment for people with developmental disabilities.

 

A high school diploma, or its equivalent.

 

WORK SCHEDULE

This position is located at a 24- hour facility.  Working Hours: 11:00 a.m., to 7:30 p.m. Regular Days Off: Vary.

 

Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service. This includes, but not limited to, acting in a responsive, professional, courteous manner with the employees, customers and public we serve at all times. Possession of valid Florida Drivers' License and acceptable driving record in accordance with Tacachale policy. Maintain Advanced Prevention and Safety Strategies (PASS) certification.
