The incumbent in this position assist in safely performing resident lift transfers by utilizing appropriate body mechanics, lifting equipment, following procedures and working collaboratively with Lift Team, Nursing, Therapies, and direct care workers. This work involves assessing residents' needs before each lift/transfer, reposition, including types of lift to be performed, condition of resident, and whether assistance is needed.

 

Daily and consistently assists in the implementation of approved and scheduled physical management programs for residents to include, but not limited to areas of daily living skills, self-care, socialization, recreation, leisure, and residents’ safety and privacy. Adheres closely to the residents’ daily activity schedule. Reports progress or lack thereof to the home supervisor and suggests improvements as possible.

 

Accurately documents residents’ information including, but not limited to behavioral issues, toileting, food/fluid intake, and active treatment following each activity as necessary. May provide valuable input concerning the observation of the resident and makes suggestions for improvements in the activities under discussion.

 

Documents services delivered and provides reports to supervisor within 24-hours.

 

Attends to residents’ injuries and illnesses by notifying the nurse or supervisor as the situation warrants. Accurately completes Incident Reports, and documents illnesses and injuries in the Shift Communication Book within one (1) hour of occurrence.

 

Attends in-service training and other staff meetings as necessary. Attends mandatory training and/or maintains certification in Prevention and Safety Strategies, Cardiopulmonary Resuscitation and Direct Care In-Service. Completes annual health screening.

 

Perform other duties as required

 

KNOWLEDGE, SKILLS AND ABILITIES 

Ability to lift up to 40 pounds.

Knowledge of the Developmentally disabled population.

Ability to learn safe proper body mechanics.

Ability to carry our verbal and written instructions for implementing prescribed treatment plans..

Ability to instruct orhters in safe proper body mechanics.

Ability to use the computer.

 

MINIMUM REQUIREMENTS

A valid Florida driver’s license (If initially hired with an out-of-state license the Florida resident must obtain a valid Florida State driver’s license within 30-days of hire. License suspended or revoked for any reason, work permits (Business purpose/ Employment/Education only license) and some types of restricted license are not acceptable. License that have “Corrective Lenses Restricted,” are acceptable, provided the driver wears the corrective lenses while operating the vehicle.

WORKING HOURS

This announcement is for four (4) part time positions. 1 FTE position will be split between two (2) individuals sharing same PSN. Total hours in workweek 20. Available Shifts: Two (2) - 7:30 a.m. to 11:30 a.m., and Two (2) - 4:00 p.m. to 8:00 p.m.  Regular Days Off: Saturday and Sunday.

 (Work schedule may be adjusted to permit flexibility in providing vital services to residents. Work on weekends, evenings, and some holidays may be required.)
