Your Specific Responsibilities:

This is a highly responsible professional position with lead responsibility for minority focused HIV/AIDS prevention, care and early intervention activities in an assigned area of the state.  It is a position that interreacts with the community and faith-based organizations as well as health Departments in 15 counties.  The primary responsibilities of this position include addressing and facilitating the HIV needs within the minority community. Fifty percent of the time is dedicated to linkage to care activities. This position is assigned to and stationed at the Florida Department of Health, Alachua County and reports to the HIV/AIDS Program Administrator for the area.

The incumbent will cause all those eligible for testing to be tested with the best means available.  The incumbent will coordinate HIV testing for Alachua County Health Department.  Input client services into EMR as required.  Ensure 1628 forms are processed according to SOH protocol and provide technical assistance on 1628 forms as needed.  Participate in outreach activities as schedule permits. Provides STD post-test counseling and schedule appointments as needed.  Manage county condom distribution for the 15-county area.

The incumbent will fill the role as Early Intervention Consultant (EIC) as prescribed by the Bureau of HIV/Aids.  Attend face to face EIC meetings or conference calls and distribute appropriate information to the staff and community.  This will include coordination of 500/501 trainings throughout area 3/13.

The incumbent will provide training, technical assistance, registration and re-registration of community and faith-based organizations interested in becoming a test site.  

Provide Linkage to care activities to ensure the engagement and retention of HIV + clients into care with the goal of improving health outcomes. This includes working with linkage to care specialists and STD program.

Assess technical assistance and training needs of community-based organizations (CBO's) to improve their capacity for providing HIV related services.  Address needs locally when possible.

Facilitate and or provide HIV training to minority CBO's.

Provide regular feedback to the HAPC on all activities.  Conduct annual workplan and monthly report of activities.

Attend all mandatory trainings and staff meetings.

Responsible for securing the designated information sets for the purposes of protecting confidentiality, data integrity, and appropriate access for all information, both confidential and public record, which is stored in hard copy and electronic forms.

This position acts as a back up to MACand hours may be adjusted to meet the needs of the community to include before, after hours and weekends.

Performs all other duties assigned.

 

Required Knowledge, Skills, and Abilities:

Knowledge of and experience with HIV/AIDS/STDs and high-risk population.

Ability to communicate effectively, verbally and in writing.

Ability to develop, execute and manage memorandums of agreement between CBO’s and FBO’s.

Knowledge of and experience with Counseling, Testing and Linkage Services

Skills in stablishing and maintaining effective working relationships with organizations, groups, and individuals.

Knowledge of the methods of data collection, analysis and report generation.

Knowledge of securing confidential information according to HIPAA.

Knowledge and experience working with an Electronic Record..

 

Qualifications:

Minimum – Training experience; ability to communicate with clients and have written and oral skills to document the requirements. Ability to work in a fluid and flexible environment to accommodate daily changing priorities. Have training experience.
