Your Specific Duties and Responsibilities:
This is an intermediate level work performing a variety of tasks under limited supervision where general policies, practices and principles can be applied to a situation not previously covered. The job requires a general knowledge of counseling techniques and social work principles and practices. Incumbents may use skill in recommending routine changes in standardized operating procedures.

Conduct interviews, provide partner services, performs field investigations, HIV Linkage to Care, PREP, NPEP, Test and Treat navigation and field testing for Syphilis and HIV, or other bacterial, biological, or viral agents.
Perform case management activities as directed in the process of identifying individuals infected with a STD, HIV/AIDS, or other bacterial, biological and/or viral agents. These activities will include but not be limited to case discussions, work prioritization, records documentation, case management, interviewing, visual case analysis, and documentation of activities in the STARS and Linkage to Care Module database.
Perform Pre and Post test HIV counseling and provides Partner Counseling Referral Services (PCRS) in accordance with STD program policy to a wide variety of individuals with or at risk of an STD, HIV/AIDS, Gonorrhea, Chlamydia, Syphilis or other bacterial, biological, and/or viral agent. Provide STD technical assistance to all Area 4 County Health Departments.
Visits and/or communicates with public and private clinics, clinicians, hospitals, laboratories, and other health care facilities to stimulate reporting of cases and positive/reactive test results in a timely manner, (2) provides information on current diagnostic and treatment guidelines, (3) acquaints clinicians with current disease trends and their role in reducing the rate of infection in the community, (4) obtains demographic, medical, or other specific client information including address, telephone number, place of employment, social security number, race , sex, date of birth, test results, diagnosis, treatment, etc.
Participate in community outreach testing events, community educational presentations, staff meetings, and training sessions.
Perform all other related duties as assigned.
Required Knowledge, Skills, and Abilities:
- 500/501 Counseling and Testing Phlebotomy.
- Computer Literacy.
- STD DISTA Training/Online Module Passport Training- completed within 6 months of hire.
- Valid unrestricted State of Florida driver’s license.
Qualifications:
- Must have recent experience two years minimum to five years maximum of conducting field investigation visits within high-risk communities and populations.
- Must have recent experience within last five years working in disease control program, social services program, clinical setting or community-based organization.
- Must have recent experience within last five years working with computer-based programs/system (i.e.: Microsoft) etc.
