The Florida Department of Health in Duval partners and collaborates with community-based organizations in the local area to provide outreach services within the community. These include, but are not limited to The Teen Health Centers, Duval County schools, afterschool programming, Teen friendly environments, etc. This position is responsible for Outreach/Prevention services and staff within the field operations program. This includes activities such as: facilitating community screening/testing opportunities; development of community collaborations and partnerships; reaching at-risk populations not easily reached through traditional intervention services and collaborating with other health providers. Some of the collaborative efforts/partnerships include, but are not limited to: The Teen Health Centers, AMI Kids, etc. Staff will performing a broad range of intervention/prevention services including screenings, prevention counseling, partner notification and linkage if needed.
Conduct interviews, provide partner services, performs field investigations, HIV Linkage to Care, PREP, NPEP, Test and Treat navigation and field testing for Syphilis and HIV, or other bacterial, biological, or viral agents.
Perform case management activities as directed in the process of identifying individuals infected with a STD, HIV/AIDS, or other bacterial, biological and/or viral agents. These activities will include but not be limited to case discussions, work prioritization, records documentation, case management, interviewing, visual case analysis and documentation of activities in the STARS and Linkage to Care Module database.
Perform Pre and Post test HIV counseling and provides Partner Counseling Referral Services (PCRS) in accordance with STD program policy to a wide variety of individuals with or at risk of an STD, HIV/AIDS, Gonorrhea, Chlamydia, Syphilis or other bacterial, biological and/or viral agent. Provide STD technical assistance to all Area 4 County Health Departments.
Visits and/or communicates with public and private clinics, clinicians, hospitals, laboratories and other health care facilities to stimulate reporting of cases and positive/reactive test results in a timely manner, (2) provides information on current diagnostic and treatment guidelines, (3) acquaints clinicians with current disease trends and their role in reducing the rate of infection in the community, (4) obtains demographic, medical, or other specific client information including address, telephone number, place of employment, social security number, race , sex, date of birth, test results, diagnosis, treatment, etc.
Participate in community outreach testing events, community educational presentations, staff meetings, and training sessions.
Performs other related duties as assigned.

The Community Relations Coordinator will collaborate with partners in the community to provide education; materials; discuss relevant issues related to morbidity/issues/outbreak situations and develop effective partnerships.

The incumbent will take the lead on educational and screening programs sponsored by community partners and/or the Field Operations Department. The programs will involve many diverse groups and the coordinator will be able to relate to all community members, community leadership, medical providers, and others. The programs will be geared to elements of the community that are difficult to reach through traditional intervention means.

The coordinator will collaborate with Surveillance and/or meet individually or in groups with Primary Care Physicians to encourage them to do annual check-ups for STDs with all age groups.
Required Knowledge, Skills, and Abilities:
- 500/501 HIV Counselling and testing
- Effective communication skills dealing with youth/adolescents
- Detail-oriented, with a reputation for thorough process documentation creation, review, and training.
- Knowledgeable in records management
Qualifications
Minimum:
- Valid Driver License
- Three years successful experience working in public health/community engagement
- Must have recent experience of two years minimum to five years maximum of working within a community-based organization providing medical services, community outreach. Conducting Health Education and/or Mental health services in high-risk areas, jails, shelters, and within homeless population gathering areas.
Preferred:
- Two years minimum to five years maximum experience conducting investigations, interviews, and/or counseling sessions that require the clients to disclose personal and sensitive information including their race, ethnicity, gender, sexual orientation, behaviors, and social economic status.
- HIV 500/501, CDC ISTDI/ASTDI, STD passport training, medical assistant and phlebotomy experience
