Your Specific Responsibilities:
As a member of the Florida Department of Health in Duval County, the incumbent will work under the direction of the program manager for the Office of Performance Improvement. The incumbent will serve as the coordinator for the Healthy Jacksonville Collaborative, the community partnership assists with the development and implementation of the Community Health Improvement Plan (CHIP). This includes planning, facilitation, and coordination of quarterly meetings for the Healthy Jacksonville Collaborative and related activities. In this role, the incumbent will coordinate evaluation and performance improvement activities related to community health assessment (CHA), CHIP, quality improvement and strategic plans, as well as local workplan activities and progress reporting for statewide initiatives for Florida Healthy Babies, Minority Health, Tobacco-Free Moms and Babies, and Healthy Communities of Florida. Additionally, the incumbent is responsible for ensuring work products meet DOH quality standards and the requirements of the national Public Health Accreditation Board (PHAB) including maintaining related documentation (i.e., standards tools, meeting agendas and minutes, presentations, etc.).
The incumbent should possess strong organizational skills to effectively prioritize work and to ensure assignments, meetings, and projects are completed in timely manner. The incumbent should have the ability to adapt with changes. The incumbent should have the ability to foster engagement and collaboration across departments as well as the ability to cultivate and grown partnerships.
This position is responsible for coordination of CHIP-related meetings and activities and maintenance of related documentation. This position is responsible for attending and participating in regular meetings for CHIP-related initiatives including Florida Healthy Babies, Minority Health, Tobacco-Free Moms and Babies, and Healthy Communities of Florida. This position is responsible for completing regular updates to key agency documents and plan including the CHA report, CHIP updates, annual progress reporting, success stories, and presentations. This position is responsible for attending meetings, sharing related information with leadership to support decision making, and for satisfactorily completing assignments and deliverables in a timely manner. This position is responsible for monitoring CHIP-related performance metrics in agency plans, the progress of CHIP-related QI projects, and for supporting CHIP-related trainings and workforce development activities across DOH-Duval departments. This position is responsible for supporting/providing informational updates for various channels (i.e. website updates, newsletters, informational flyers, etc.). This position is responsible for fostering team collaboration, supporting a common vision, and creating momentum toward action and achievement of performance targets within established timeframes.

The incumbent will perform other miscellaneous job-related duties, as assigned. This may include supporting the Organization in disaster and emergency management and response functions. Work is primarily conducted within the offices of Florida Department of Health in Duval County. The hours of operation are Monday through Friday from 8am to 5pm, excluding state holidays and office closures. Telework is not available.

Required Knowledge, Skills, and Abilities:

Qualifications
Minimum:
- At least 2 years’ experience with community planning and working with stakeholders and community organizations.
- At least 2 years’ experience of developing written reports, plans, and presentations containing community data and technical information (e.g. community health status report, progress reports, annual reports, etc.)
- At least 2 years’ experience with communicating information and facilitating meetings, giving oral presentations, and managing virtual meetings in a professional manner.
- At least 4 years of related work experience
- Bachelor’s degree or master’s in public health, Community Health, Health Education or a related field from an accredited college or university can substitute required work experience on a year-for-year basis.
Preferred:
- Certification in areas such as Certification in Public Health (CPH), Project Management Professional (PMP); Certified Health Education Specialist (CHES), Certified Professional in Healthcare Quality (CPHQ); and/or Lean Six Sigma (i.e. Black belt).
