This is intermediate-level work performing a variety of tasks under limited supervision where general policies, practices and principles can be applied to situations not previously covered. The job requires a general knowledge of client centered counseling techniques and social work principles and practices. Incumbents may use skill in recommending routine changes in standardized operating procedures. This position is responsible for health education counseling as a part of the STD and HIV Counseling, testing, and linkage to care, treatment, PrEP, or other activities within the Division of Disease Control, STD and HIV Program. Service delivery may occur within the CC Clinic, Mobile Medical Unit, or other settings in the community. This position addresses priority risk factors and prevention, including PrEP, Rapid Access to Treatment (Test and Treat), prevention with Persons living with HIV. This position will participate in outreach screening and community education activities. Responsibilities will be performed in compliance with Disease Control Programs and timeline.

Public Health Competency Domain 2: Policy development/Program Planning Skills

  • Plans, coordinates, implements, and evaluates evidence-based HIV health education and prevention activities, with the program supervisor, for individuals and populations at risk for HIV and persons living with HIV.
  • Provides health education regarding HIV prevention, PrEP, nPEP, HIV testing, treatment, linkage to care, viral suppression, risk reduction, and prevention of HIV transmission.
  • Coordinates implementation of PrEP, nPEP, Test and Treat, HIV self-testing, and other HIV prevention initiatives within area 4.
  • Continually works to develop strategies to recruit and retain target population in health education programs
  • Provides technical assistance to staff
  • Attend professional development trainings to maintain and enhance professional skills.  Attend internal and external meetings as necessary.

  • Makes referrals to case management, financial and mental health counseling.

  • Refers clients to appropriate services related to continuum of care including but not limited to clients who are lost to care, lost to follow-up after initial visits, re-establishment of services/education, and referral to community-based organizations to promote continuity and coordination of care across various healthcare services.
  • Assists in educating the community about local department services, and condom distribution as needed.
  • Work with the program supervisor and other Department of Health team members to engage businesses and churches in Fath-Based Response to AIDS (FRTA’s) and Business Response to AIDS (BRTA’s).
  • Provide input and review workplans, Integrated HIV prevention and care plan, ending the HIV epidemic plan to ensure effective strategies and activities are planned and measurable to impact HIV incidence.
  • Works with local coalitions groups and organizations in identifying community problems, prioritizing issues and identifying resources.

  • Responsible for HIV prevention for HIV+ persons and risk reduction to HIV/AIDS patients of the Florida Department of Health in Duval County. Meets with individuals and provides education and guidance that may lead to safer sex behaviors and prevention of continued transmission of HIV, STDs and other communicable diseases. Works cooperatively with other health educators, DOH-Duval units and outside agencies as needed.
  • Records data on all activity into appropriate data system HMS, STARS, LTC Module, and EPIC) on a regular basis. Submits monthly CHD Monthly Service Delivery report to the Clinical Operations Manager by the 10th of the month following service delivery.
  • Perform other duties as assigned. Participate in Disaster Preparedness trainings and activities. Perform travel in relation to above duties and for professional development. Ensures accurate and timely submission of Employee Activity Records (EARS), employee leave and attendance records, travel log and monthly statistics to supervisor/assigned designee, and program coordinator as applicable. Perform other related duties as assigned. Maintain a courteous and cooperative working relationship with clients, the community and other internal and external customers.

Qualifications:

Minimum:

  • Experience working in public health, health education, disease control programs, clinical setting, or two or more years' experience working in a community-based organization or social services programs
  • Experience of educating clients about HIV, including HIV transmission, prevention, treatment, and the importance of staying engaged in medical care
  • Experience educating clients about injectable HIV medications or other long-acting HIV treatment options
  • At least 3 years of experience working with individuals diagnosed with HIV/AIDS
  • A current, valid, and unrestricted driver’s license

Preferred:

  • Candidate with a bachelor’s degree or higher in Public Health, Health Education, Health Sciences, or a related field with experience working in public health, health education, disease control programs or clinical setting
