The duties and responsibilities are those of public health nursing position assigned to provide advanced, comprehensive public health nursing services in the Tuberculosis (TB) Program. Utilizing the public health nursing process, the incumbent will be required to independently conduct, coordinate and monitor for quality control and assurance. Work will be performed under the general supervision of the Assistant Community Health Nursing Director.

Public Health Competency Domain 1: Analytical/Assessment skills

  • Collects, compiles, and analyzes client related data to prepare weekly, monthly and annual program or agency reports.
  • Maintains records and reports, documenting service and care provided as required, in both paper and electronic formats.
  • Provides advanced public level assessment of health status and health care needs of individuals, families and/or groups of individuals (particularly those with complex and multiple problems and communicable disease) in a variety of settings, including but not limited to clinics, schools, hospitals, homes, and other public/community facilities.

  • Accurately applies nursing principles and techniques in the direct care and treatment of women’s health, family planning and/or pediatric patients
  • Provides direct patient care along with in-depth teaching, guidance, counseling and advisement that is directed toward the prevention, promotion and maintenance of optimal health. 
  • Formulates and initiates comprehensive case management for individuals and/or families served which includes follow-up.
  • Develops, implements, monitors, and modifies plans of care for individuals in various settings through an interdisciplinary and collaborative process with other members of the TB Program team.
  • Provides follow-up activities to ensure continuity of care until clients achieve maximal treatment outcomes.
  • Positively assists provider in clinic session.  Interprets and implements provider’s orders and recommendation to patients appropriately.
  • Properly obtains vital signs and medical histories; and performs simple laboratory procedures (finger stick/heel stick).
  • Willingly and proactively assists in maintaining inventory of medical supplies and medications; and prepares examination/treatment rooms and equipment.  Occasional provision of vaccines, FLSHOTS data entry, and inventory maintenance.
  • Provides health education.
  • Provides advanced public level assessment of health status and health care needs of individuals, families, and/or groups of individuals (particularly those with complex and multiple problems and communicable diseases) in a variety of settings, including, but not limited to, clinics, schools, hospitals, homes, and other public/community facilities.  Provides direct patient care along with in-depth teaching, guidance, counseling, and advertisement that is directed toward the prevention, promotion, and maintenance of optimal health.  Formulates and initiates comprehensive plans of care for individuals and/or families served which includes follow-up.

 

Public Health Competency Domain 5: Community Dimensions of Practice skills

  • Coordinates and links clients with other health and/or social service agencies to facilitate movement of clients through the public health system. 
  • Serve as a liaison between individuals and families in need of care and other service agencies in the community.  Serves as a key liaison between other healthcare providers, including hospitals, private medical providers and other public healthcare facilities, and DOH-Duval.
  • Promotes cooperation within the agency and externally maintaining positive relationships with Management Team, Physicians, Advanced Practice providers, nurses, Health support Technicians, Human Service Counselors, Senior Clerks and other staff.  Develops and maintains interdisciplinary team approach to patient care and case management.  May participate in quality improvement activities, including the peer review process.

  • Participates in educational, professional and community activities that relate to professional growth and aids in the improvement of the delivery of client care.
  • Conducts TB related community presentations that promotes and increases public awareness of the need and benefits of participation in health promotion/prevention activities.
  • Performs other related duties as assigned by the Asst. Director of Nursing or designee.  Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.

Qualifications:

Minimum:

  • Licensed as a Registered Nurse in accordance with Florida Statute 464 or eligible to practice nursing in accordance with Florida Administrative Code 210-8.27.
  • Experience in case management and/or triage
