This professional position is responsible for providing medical case management services through education, counseling, patient advocacy, and coordination of services to individuals medically managed through the Department of Health Clinic/Ryan White Program. Under the direction of the Regional Linkage Coordinator, the Medical Case Manager is responsible for linking clients diagnosed with HIV access to health care and long-term case management services.
Link recently diagnosed HIV-infected patients to medical care and case management services. Builds rapport and trust with patients and engages patients. Integrates social and medical services in a holistic approach to patient care. Assists patient in accessing services and coordinates and tracks implementation of services. Provisions of services may require work in the community, home visits, outreach events, and/or satellite offices as determined to meet the need of the program and the client. Ensure completion of the required intake procedures per the Florida Department of Health’s HIV/AIDS case management standards and guidelines. Communicate with clients face-to-face, over the phone, and electronically. Assist clients on assigned caseload with navigating the Ryan White system and serving as their main point of contact adhering to Ryan White Case Management Standards in all job duties. Develop and maintain an understanding of available community-based resources. Ensure the linkage of clients with needed programs such as Supplemental Security Income, Food Stamps, Medicaid, Medicare, and Community Long Term Care (CLTC).
Document case management notes, services, lab results, medication and other required documentation on paper source and/ or electronic database. Participate in required program trainings and staff meetings. Provide coverage for other medical case managers as needed. Develops individualized service plans for assigned patients based on psychosocial assessment of patient needs. Provides ongoing follow-up and support to assist patient retention in programs related to medical care, including, but not limited to, contacting patients in reference to missed/ upcoming appointments. Maintains required paperwork for federal, state and private assistance programs within the framework of state and federal program guidelines and requirements. Maintains an accurate database of services, contact information, and other required patient documentation in Provide Enterprise software system. Submit monthly reports as assigned.
Attend monthly RW Medical Case Management Co-Op meetings. Conduct comprehensive client needs assessments and develop corresponding care plans. Coordinate services with community programs, vendors, and health care providers for client services. Follow up on client appointments, referrals, and authorized services. Maintain organized client files with required documentation.
Perform all other duties as assigned pertaining to job.
Required Knowledge, Skills, and Abilities:
- Knowledge of HIV, HIV prevention, linkage and adherence
- Knowledge of community resources for people living with HIV, including the Ryan White Program and HOPWA
- Knowledge of Microsoft Outlook, Word, Excel and PowerPoint
- Knowledge of HIPAA and security compliance
- Excellent verbal and written communication skills
- Excellent time management, organizational, and coordination skills

Qualifications
Minimum:
- Experience working in a Federally Qualified Health Center.
- Experience verifying medical insurance electronically.
Preferred:
- Experience with Electronic Medical Records
