Your Specific Responsibilities:

This is advanced-level work performing or supervising a variety of complex tasks under limited supervision where general policies may not have been previously established and standard methods may not be available.  The job requires an extensive knowledge of counseling techniques and social work principles and practices. Incumbents may use skill in developing methods and procedures for special applications or functions.

The Social Services Counselor serves as a member of an interdisciplinary team in a care coordination concept for a defined high risk maternal and child population.  Responsibilities include participant education regarding risk status as well as the provision of case management/care coordination to reduce defined risks.  The employee performing these duties does so in the context of client centered service delivery and in coordination with other professional team members

Contacts program participants in home or other community locations (to include the jail).  This position is responsible for providing case management/care coordination services via an approved curriculum to pregnant women, infants, and their families who have been determined eligible for participation in Healthy Start (This may include (SEN) Substance exposed pregnant mothers and their newborns).

Provides program participants with pathway screenings based on Healthy Start Standards and guidelines (HSSG), education, information, referrals and assistance in accessing services. Explores local resources and meets with alternative service delivery providers to promote access to health care through community agency collaboration.  Establishes liaison with community agencies to facilitate referrals and ensures the effective coordination of service delivery to clients.  Attends client case staffing as designated.

Documents all contacts and case involvement with clients and referral agencies in WFS for the purpose of follow-up and continuity of care.  Completes administrative records and reports as required.  Completes assessments, family support plans, progress notes and referrals in accordance with Healthy Start Standards and Guidelines and DOH-Duval Protocol.

Attends staff meetings, in-service trainings, and professional development sessions as required.  Ensures accurate and timely submission of Employee Activity Records (EAR’s), employee leave and attendance records, travel logs, monthly reports to assigned supervisor.  Performs other related duties as necessary.

Maintains a courteous and cooperative working relationship with clients, the community, and other internal and external customers.

Use of your personal vehicle is required to complete the face-to-face visits.

Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.  Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. All employees are expected to complete timesheets and EARs/DARs as required, both accurately and within the required timeframe. All employees are expected to complete mandatory trainings within the required timeframe, as deemed necessary by the Department, the Delegated Authority, grant requirements and/or Florida Statutes.


Required Knowledge, Skills, and Abilities:

Knowledge, skills, and abilities, including utilization of equipment, required for the position:

Ability to plan, organize and coordinate work assignments; knowledge of counseling techniques and practices; knowledge of the principles and practices of social work, public health, nursing, maternal, child health, education or counseling; knowledge of methods of compiling, organizing and analyzing data; ability to follow through on all phases of the diagnosis, treatment and rehabilitation of clients; ability to provide casework services to clients including assessments/evaluations/prevention/treatment services; ability to collect, analyze and interpret data; ability to communicate effectively; ability to establish and maintain effective working relationships with others, knowledge of computers and applications such as Microsoft Office, PowerPoint, Excel and other similar.



Must have a bachelor’s degree in Social Sciences from an accredited college or university: as per Healthy Start Standards and Guidelines, provider qualification per chapter 6.

Must have case management experience or experience working with families and children.

Must have a valid driver’s license.
